Copilot review round 1.
- `newAttemptId` calls `globalThis.crypto.getRandomValues(...)` directly
rather than through a bound alias. CodeQL's `js/insecure-randomness`
browser model recognizes a secure RNG by that literal method call, so
the indirection would have left the `Math.random` last resort
classified as an unmitigated source.
- The no-crypto test arm now removes `globalThis.crypto` entirely
instead of hiding its two methods, matching the crypto-absence test in
`src/test/core/auth/utils.test.ts`. Hiding only the methods left the
global truthy, so the arm could not have caught a regression that read
`globalThis.crypto` without the optional guard. `withoutCryptoMember`
narrows to `randomUUID`, its only remaining caller.

`Record<string, unknown>` does not overlap `Crypto` (TS2352 under `tsc -b`).
`Partial<Pick<Crypto, …>>` says the same thing in one legal cast and keeps
`delete` operating on a known-optional property.

Both findings from the v2.5.0 milestone-merge review (#2215).
1. `progressToastId` collided across distinct progress streams.
`ProgressToken` is `string | number`, so `String(token)` mapped the
numeric token 7 and the string token "7" onto one id — and because
notifications keyed by the same id are *replaced* rather than
stacked, two concurrent streams overwrote each other's toast. The
absent case was worse: it hardcoded the sentinel "default", which a
server is free to send as a genuine string token. The id now carries
the token's type (`progress-n:7` / `progress-s:7`) and gives the
no-token case a prefix of its own (`progress-none`) that no token
value can produce.
2. `newAttemptId`'s fallback now prefers `crypto.getRandomValues`.
`randomUUID` needs a secure context; `getRandomValues` does not and
exists in every browser that has `crypto` at all — so the exact
situation the fallback exists for (a `file://` page, a plain-HTTP
non-loopback host) still has a CSPRNG on hand. `Math.random` stays
as the last resort for a `crypto`-less global, and the "never a
security token" comment stays too. Retires CodeQL alert 72
(`js/insecure-randomness`) honestly rather than by dismissing it.
Tests: collision cases for 7 vs "7" and the absent token vs "default" /
"none", a distinctness sweep over the whole id space; and the attempt-id
fallback test split into a getRandomValues arm (asserting the CSPRNG is
used and `Math.random` is not) and a no-crypto-at-all arm.

- skills.ts: apply the two RFC 3986 §6.2.2 percent-encoding normalizations
the URL parser does not — decode an escape standing for an unreserved
character, and upper-case the hex of every escape that remains. `URL.href`
leaves `%72eference.md` encoded, so a server echoing an RFC-equivalent
form of the URI we asked for was rejected by `onReadSkillFile` as a
different resource, and an encoded name segment produced a false
`name-path-mismatch`. Both are the tool calling a conforming server wrong.
- oauthResume.test: assert the Skills restore path. It was wired into the
setters but never checked, and the comprehensive present-value and
undefined-fallback cases omitted Skills entirely. Adds it to both, plus a
case restoring a REAL saved selection rather than the EMPTY value — a bug
that always wrote the default would have passed the others.

Adds detection, enumeration, and conformance checking for the Skills
extension (`io.modelcontextprotocol/skills`) — phases 1 and 2 of #2234,
which is everything its Acceptance list names.
Skills is a *server*-declared extension, read off the connecting server's
`capabilities.extensions`, so it is deliberately absent from
`ADVERTISABLE_EXTENSIONS` — that registry is what the Inspector advertises
and the user toggles, and an entry there would be a meaningless toggle.
`skills/list` and `skills/get` are consumer-owned extension methods that
neither era codec defines, so the SDK's era gate skips them and they go out
as ordinary `client.request` calls with explicit result schemas. The
raw-wire channel modern `tasks/*` needs is not used, and the Skills tab is
not era-gated: a legacy-era server that declares the extension is serving it.
The checks are the point, not the list view. `core/mcp/skills.ts` reports
each obligation SEP-2640 states — the name/path invariant, the digest
format, the 512-entry and 16 MiB limits, and `resources: "dynamic"`, which
means integrity cannot be verified at all. Digest verification hashes the
fetched bytes with WebCrypto and returns a mismatch with both digests
attached rather than throwing, because showing a mismatch loudly is the
whole value proposition.
Files are fetched on demand: SEP-2640 is explicit that a `resources/read`
of a `SKILL.md` is not a load and confers no standing, so none of the SEP's
host machinery is implemented.
The `skills-http` fixture serves four skills over two pages, three of them
deliberately non-conforming — without those the verification code is
untestable.
Phase 3 (CLI, TUI, `resources/directory/read`, paged mode) is #2248.

Copilot round 4:
- the EMA `interactive` arm kept `pendingStepUpRetryRef`, on a comment
claiming it was "still on its way to a redirect". That is wrong twice:
`prepareOAuthRedirect` returns before the navigation is resolved, so a
pre-navigation failure leaves the operation for a later, unrelated
step-up to re-run; and the full-page navigation destroys the closure on
the success path anyway. It now clears the ref, matching the standard
step-up below.
- the two surviving `void (async () => …)()` sites now state why the
caller cannot await, as AGENTS.md requires — the comments had described
only what the catch handles.
- the `randomUUID` fallback test restored an own descriptor that never
existed (it is inherited from `Crypto.prototype`), leaving an own
`randomUUID: undefined` that would push every later test in the file
onto the fallback path. It deletes the temporary property instead, and
asserts the global is a function again afterwards.

Copilot round 3, four items:
- the snapshot "attempt token" was the stored serialization, which is not
unique: the snapshot records neither the authorization URL nor its
`state`, so two concurrent redirects for one server with unchanged UI
serialize identically. `OAuthResumeSnapshot` now carries an optional
`attemptId`, stamped per write and matched on clear. Optional because
an older build's snapshot, mid-redirect across an upgrade, has none —
and treating that as a match would strand a live callback.
- the resume-failure toast promised a retry even when the session had
ended and nothing was restored. Copy is now chosen by whether anything
is left pending; `authRecoveryAbandonedMessage` covers the other case.
- a connect-scoped redirect that failed left the connection at
"connecting" with the active-server lock held, since the redirect was
what would have ended the attempt. It now disconnects first, as the
sibling arm in `useConnectionLifecycle` does — scoped to that context,
because an ambient or step-up failure does not mean the session is dead.
- both terminal EMA step-up failure arms now drop the stored retry. A
later step-up does not overwrite the ref, so a retained operation could
be re-run by an unrelated authorization. The `interactive` arm keeps
it: that one is still on its way to a redirect.

Copilot round 2, both findings valid:
- `resumePendingReauth`'s restore could resurrect a challenge whose
session ended during the awaits — a server switch or disconnect clears
the slot on purpose. It is now gated on the attempt still targeting
the live session, client identity included (reconnecting to the same
server builds a replacement client an id-only check would admit), and
uses a functional update so a newer deferral queued in the same batch
wins rather than being read past.
- `prepareOAuthRedirect` cleared whichever resume snapshot was current,
not the one it wrote. Nothing single-flights that callback, so a
redirect failing before navigation could delete a later attempt's
callback-routing state. `writeOAuthResumeSnapshot` now returns the
serialization it stored, and `clearOwnOAuthResumeSnapshot` removes it
only while it is still the current one.

A resume snapshot written before this change carries `selectedToolName`
where `ToolsUiState` now expects `selectedToolKey`. The blind cast in
`restoreTabUiFromSnapshot` carried it through as a stray field that would be
re-serialized on the next redirect. Route the Tools entry through a
`normalizeToolsUi` that drops it explicitly instead.
The name is deliberately not migrated to a row key: a snapshot lives for one
redirect (so this only arises when the app is upgraded mid-flight), and the
tools list is fetched after reconnect, long after restore — there is nothing
to resolve the name against. The selection is lost; search, form values, and
the run-as-task toggle restore intact.

#1961 keyed the sidebar's React children by source position, but everything
downstream still identified a tool by `tool.name`. With a `tools/list` that
repeats a name, both copies highlighted at once, the click guard made the
other copy a no-op, and `tools.find((t) => t.name === …)` always resolved to
the first — so a later copy rendered but could never be opened.
Thread the row key through selection instead:
- `utils/toolUtils.ts` now owns `toolRowKey` (moved out of ToolControls) and
adds `findToolByRowKey`, which compares computed keys rather than parsing
one so the two can't drift.
- `ToolControls`: `selectedName` → `selectedKey`; the highlight and the click
guard compare keys, and `onSelectTool` emits the row key.
- `ToolsScreen`: `ToolsUiState.selectedToolName` → `selectedToolKey`, with
both lookups going through `findToolByRowKey`. Renamed through
`screenUiState.ts` and `App.tsx`'s `onToolsUiChange`.
`onCallTool` still sends `selectedTool.name` — the duplicated name is the
genuine wire identity; only the UI identity needed to be unique.
Also prefixes the excluded (SEP-2243) rows' keys: they render into the same
`Stack` as the list rows but index within their own list, so an excluded tool
sharing a name and index with a listed one collided in the key space.

* refactor(web): sharpen src/lib vs src/utils rule and move misfiled modules (#1776)
Codify the empirical rule the web client already followed loosely — utils =
pure functions that compute; lib = things that instantiate, adapt, or touch
the environment — in AGENTS.md and clients/web/README.md, and move the modules
that sat on the wrong side of it.
Moves (source + co-located test):
- lib/sandbox-csp -> utils (pure CSP-string construction; type-only import)
- utils/browserTabVisibility -> lib (registers visibilitychange listeners)
- utils/oauthResume -> lib (reads/writes sessionStorage)
- utils/clearServerOAuthState -> lib (drives live InspectorClient / OAuthStorage)
Borderline audits (kept in place): pendingReauth (pure type declarations),
deepLink / correlateTransportErrors / mcpNetworkHeaders / oauthUx / oauthFlow /
oauthNetworkPhase (pure transforms / type-only core imports) stay in utils;
downloadFile stays whole in lib (dominant side is DOM side effects).
Also relocate the lone src/test/lib/downloadFile.test.ts straggler beside its
source at src/lib/downloadFile.test.ts, and document the test-placement rule
(side-by-side by default; src/test/ only for core/ tests, the integration
project, and shared scaffolding).
Pure move — no behavior change; no build/coverage config keys off the boundary.

* refactor(web): address review — fix stale paths, move customHeaders, one-way arrows (#1776)
Responds to the @claude review on PR #1784:
1. Stale path references (fixed): update the four in-repo pointers left at
pre-move paths — specification/v2_auth_mid_session.md (oauthResume,
browserTabVisibility → lib/), and the two load-bearing sandbox security
comments (sandbox_proxy.html, sandbox-controller.ts → src/utils/sandbox-csp).
2. lib/types/customHeaders (moved): it's pure (types + pure transforms), the
rule's clearest counterexample. Moved lib/types/customHeaders.{ts,test.ts} →
utils/ (no external importers; only its own test) and removed the
undocumented lib/types/ subdir. Documented that utils/ is the home for pure
shared domain types + their constructors — no separate types/ bucket.
3. One-way arrows (item 3): declare OAuthResumeAuthKind in utils/pendingReauth
and re-export it from lib/oauthResume, so the only cross-directory arrow now
points lib → utils. Codified that invariant in AGENTS.md.
Wording nit: AGENTS.md + README now state diagnostic logging
(console.warn/error) doesn't count as a side effect for the utils rule.

* docs: address review round 2 — stale customHeaders refs, rule wording (#1776)
Doc-only follow-up to the round-2 @claude review on PR #1784 (no code changed):
1. Stale paths (same class as round-1 item 1): the customHeaders move left
three references to the deleted lib/types/customHeaders.ts path in
specification/ (v2_ux_interfaces_phase_0_4_inventory.md,
v2_ux_interfaces_plan.md ×2) — updated to utils/customHeaders.ts.
2. customHeaders anchor: note in AGENTS.md that its shape is staged for
ServerSettingsForm (per v2_ux_interfaces_plan.md), so its zero-importer
state reads as intentional rather than rot; kept as the no-types/-bucket
illustration and led the anchor list with load-bearing modules.
3. Type-only-import clause: split the conflated clause — a type-only import
(pendingReauth) and re-exporting core's pure functions/constants
(oauthUx/oauthFlow) are distinct reasons to stay in utils; what makes a
module lib is wrapping core's stateful runtime. Fixed in both AGENTS.md
and the web README.
Cosmetic: re-padded the v2_auth_mid_session.md table row to column width.

* docs: address review round 3 — scope the "no types/ bucket" claim (#1776)
Doc-only follow-up to the round-3 @claude review on PR #1784 (no code changed):
1. "no separate types/ bucket" overreached: clients/web/src/types/ does exist
(navigation.ts + react-syntax-highlighter .d.ts ambient stubs wired via
tsconfig.app.json paths). Scoped the claim in both AGENTS.md and the web
README to "no types/ sub-bucket INSIDE lib/utils", and noted src/types/ is
for ambient .d.ts stubs, not domain types.
2. README example list drifted from AGENTS.md's round-2 load-bearing anchors —
realigned it (inspectorTabs/mcpNetworkHeaders, dropped the zero-importer
customHeaders from the headline list).
3. Clarified v2_ux_interfaces_plan.md:185 so the v2 path reads as the copy's
destination, not as the v1.5 source location.
Follow-up filed as #1785 (board Todo): src/types/navigation.ts InspectorTab is
dead and duplicates utils/inspectorTabs.ts — kept out of this pure-move PR.

* docs: address review round 4 — reconcile src/types/ claim with #1785 (#1776)
Doc-only follow-up to the round-4 @claude review on PR #1784 (no code changed):
The round-3 scope fix overshot: it asserted src/types/ holds "not domain types
at all", but the dead navigation.ts InspectorTab is a plain-.ts domain type
still living there, and specification/v2_ux_interfaces_plan.md:181-182 actively
prescribes placing InspectorTab there — so AGENTS.md and that spec doc gave
opposite guidance about the same directory.
- AGENTS.md + web README: softened to "not the place for new domain types"
and added a parenthetical that the one lingering domain type (dead
navigation.ts) is tracked for removal in #1785; until then a pure domain
type belongs in utils/, not src/types/.
- v2_ux_interfaces_plan.md:181: marked the tab-enum placement guidance as
superseded by #1785 (live type is InspectorTabId in utils/inspectorTabs.ts).

* docs: address review round 5 — fix garbled #1785 clause, restructure utils rule (#1776)
Doc-only follow-up to the round-5 @claude review on PR #1784 (no code changed):
1. Grammar (item 1): the round-4 #1785 clause read "superseding [#1785] will
remove it" — a participle with no subject. Reworded to "the dead
navigation.ts InspectorTab, which #1785 will remove".
2. Readability (item 2): the src/utils/ bullet had accreted into a single
~1,400-char blob across five review rounds. Promoted the carve-outs to
scannable sub-bullets (Domain types / Diagnostic logging / Importing from
core) and lifted the src/types/ note out to its own line, since it's a
statement about a sibling directory, not about utils.
AGENTS.md only; the README's parallel sentence was already clean.

* docs+config: address review round 6 — correct the coverage-glob claim (#1776)
Responds to the round-6 @claude review on PR #1784:
1. "no build/coverage glob" was factually wrong (should-fix): the web coverage
include in clients/web/vite.config.ts DOES enumerate src/lib/** and
src/utils/**. The conclusion (a move between them is coverage-neutral)
survives, but the reason was wrong. Reworded both AGENTS.md and the web
README to say nothing *enforces* the boundary — no path alias, and the
include lists both dirs so a move is coverage-neutral — plus the important
whitelist caveat: a module outside components/lib/utils/server falls out of
the ≥90 gate entirely.
2. Removed a pre-existing duplicate glob in vite.config.ts (src/lib/**/*.{ts,tsx}
appeared twice, lines 161 and 163). Inert (vitest de-dupes) but it sat three
lines from the config the doc now describes.
Nits: gave the README bullet the same round-5 sub-bullet restructure as
AGENTS.md, and made its #1785 reference a real markdown link (bare #N doesn't
autolink in rendered repo markdown).
npm run ci green (config change re-verified through coverage + smokes + Storybook).
