4429c0c0bf fix(auth): keep the failure path total, and align the docs with the new order (#2144)
Copilot review round 14. Two code fixes and a documentation sweep.

- `buildRevocationRequest` ran outside `revokeToken`'s best-effort try.
`encodeURIComponent` throws on a lone UTF-16 surrogate — valid JSON, so it
can reach there from a persisted client id or secret — and every caller has
already cleared its local state by then, so a rejection instead of a `failed`
outcome would break the documented guarantee. Construction is inside the try
now, and the catch reports `params.endpoint` rather than a `url` that may not
  exist on that path.
- The fixture's Basic decode turned a malformed percent escape into an Express
500, reporting a bad credential as a server fault rather than as the
`invalid_client` 401 the endpoint means. Decoding failure is failed
  authentication.
- Ten places still said revocation happens *before* the clear, which the
reorder in the previous commit made false: the settings-form description, the
`--no-revoke` help text, both READMEs, `docs/mcp-server-configuration.md`,
three JSDoc blocks, and the PR body. Swept, and the ordering is stated where
it is load-bearing rather than asserted in passing. Also merged the two
  adjacent JSDoc blocks on `InspectorClient.clearOAuthTokens`.

56ac83051b fix(auth): RFC-encode the Basic credential; make the TUI clear await its work (#2144)
Copilot review round 6.

- The Basic credential is now form-urlencoded per RFC 6749 §2.3.1. Round 1
matched the SDK's raw `id:secret` on the theory that a credential should be
presented the same way here as at the token endpoint; that does not hold — a
client authenticated with `client_secret_post` never exercised the SDK's
Basic path, so there is no precedent to match, and the raw form is ambiguous
for an id containing `:` and makes `btoa` throw on a non-Latin-1 secret. The
  fixture decodes each half.
- The ctx-less grant read is suppressed on the store's issuer *stamp* rather
than on the token's value. The active slot can hold client information
without a token, in which case the read falls back to the legacy grant — and
a value collision with any other issuer would have dropped that grant
  unrevoked and unreported.
- The TUI announced "OAuth state cleared" on the keypress, so with a bounded
revocation request it said so while the work was still in flight and then
  removed it when the work finished. `onClearOAuth` returns a promise;
`AuthTab` shows "Clearing OAuth state…" until it settles, ignores repeats
while one is in flight, and skips exactly the revision bump its own clear
caused so the confirmation is not immediately reset by it.
- `handleClearOAuth` now claims an attempt and retires a superseded one, the
way `handleDisconnect` does: the await can keep it suspended for seconds,
long enough for the user to switch servers and have server A's outcome —
including its disconnect — land on server B.

781aa69d19 fix(auth): revoke every issuer-bound grant, not just the active one (#2144)
Copilot review round 3.

`clear(serverUrl)` deletes EVERY `byIssuer` slot, but revocation read only the
context-free (active-issuer) token. A server that had authorized against
issuers A and B therefore revoked B, then destroyed the local record of A while
A's grant stayed live at its authorization server — the same leak this feature
exists to close, one level down.

- `OAuthStorage.listIssuers(serverUrl)` is the new seam (one implementation:
every backend extends `OAuthStorageBase`). `collectGrants` walks it plus the
ctx-less read, deduping by token, so every grant the clear will delete is
  covered.
- Metadata is cached once per server, not per issuer, so a grant bound to an
issuer the cached document does not describe cannot be revoked: sending its
token to that endpoint would hand a credential to a server that never minted
it. That grant is reported as failed — dropped unrevoked — rather than
  silently discarded or misdirected.
- `aggregateOutcomes` gives a failure precedence over another grant's success,
so one live grant cannot hide behind another's 200. Exported for its own
test, since `computeOutcome` returns early on the empty case.

Also from that round:

- The fixture's /oauth/revoke now verifies token ownership (RFC 7009 §2.1).
Access tokens record their owning `client_id`; a token belonging to another
client is left alone and still answered 200 (§2.2 — the response must not
tell one client whether another's token exists). New e2e case proves a
second registered client cannot revoke the first's grant.
- The TUI rendered the revocation-failure note in cyan, the informational
tone. It is a partial success — the local clear really happened — so it is
not an `error` status, but the grant may still be live and cyan understated
that. `AuthTab` takes an `oauthMessageTone`; the message says so explicitly.

7f27d7e9c7 fix(auth): address Copilot review round 1 on the RFC 7009 revocation (#2144)
- RFC 8414 §2 defaults an omitted `revocation_endpoint_auth_methods_supported`
to `client_secret_basic`; it does not inherit the token endpoint's list.
Inheriting made metadata advertising only `client_secret_post` there send
POST credentials to an endpoint that never advertised the method.
- The 5s timeout was inert on the web paths: `createRemoteFetch` re-issues the
call as a POST to `/api/fetch` and drops `init.signal`, and the backend's
outbound fetch gets none either, so a wedged AS could hold the teardown open
indefinitely. A wall-clock race now enforces the deadline; the signal is kept
because it genuinely cancels the direct-fetch paths.
- The settings-modal Clear read `settingsModalTarget`, which comes from the
persisted `servers` list, while edits live in `settingsDraft` until the save
debounce. Toggling the checkbox and clearing immediately used the previous
value. It now merges the draft.
- `--relogin` clears both key spellings, so it now revokes from both rather
than stopping at the first populated one — a stale entry under the other
spelling is a live grant. The normalised key goes first, matching
`findStoredServerState`, and a second key holding the same token is skipped.
- The fixture's /oauth/revoke now ENFORCES RFC 7009 §2.1 client
authentication (both RFC 6749 §2.3.1 forms), so the e2e's claim that the
Inspector authenticates correctly is actually tested; two negative cases
assert a wrong secret and an unidentified client are refused.
- Dropped the `as unknown as` casts from the tests: a typed `InspectorLogger`
double, and a spy on the real storage instead of a spread-and-cast.
- Added CLI-level tests through `runCli` for `--no-revoke`, the per-server
`oauth.revokeOnClear`, and the stderr warning, and TUI tests for the
forwarded opt-out and the rendered failure.

56d3c04643 fix(test-servers): address review on the cancellation showcase
- `slow_task` sleeps through a helper that removes both listeners on
whichever outcome wins and returns early on an already-aborted signal.
`{ once: true }` detaches on the event firing, not on the waiter losing
interest, so the old loop left one listener per second attached and a
  60s run tripped MaxListenersExceededWarning.
- Report the outcome on stderr. A successful cancel closes the stream the
result would travel on, so the tool's return value is undeliverable by
construction — the README now points at the server's terminal rather
than at a result panel that cannot show it.
- Drop a double cast the file's own next hunk shows is unnecessary, and a
floating observer promise whose assertion was false anyway: `postSend`
rethrows before awaiting the SSE wait, so the send rejects on abort
whether or not that wait was cancelled.

85c5206895 fix: cancel a Streamable HTTP call by closing its stream (#2140)
The Cancel button POSTed `notifications/cancelled` on every transport. On
a 2026-07-28 Streamable HTTP connection that is the wrong signal — the
spec makes closing the request's SSE response stream the cancellation
signal there and says the notification is neither required nor expected —
so a spec-compliant server acknowledged it 202 and dropped it while the
tool ran to completion.

The SDK already implements the fork, off `transport.hasPerRequestStream`.
Every Inspector connection is wrapped in `MessageTrackingTransport`, which
did not forward that flag, so the SDK read `undefined` and took the stdio
branch on every client — the CLI and TUI included, whose real
`StreamableHTTPClientTransport` advertises it correctly.

The web client needed three more links: its browser transport answers for
the upstream transport that lives on the backend, so it advertises the
flag for `streamable-http` and applies the SDK's `requestSignal` to its
`POST /api/mcp/send`; and `/api/mcp/send`, held open for the whole call,
forwards the browser's disconnect to the upstream `transport.send`.

The test fixture's `toWebRequest` built a Web Request with no `signal`, so
no fixture server could observe a disconnect at all — without that, a test
asserting the cancel reaches the server passes vacuously.

stdio and SSE keep `notifications/cancelled`: they multiplex every request
over one shared channel and have no per-request stream to close. So does
the legacy era, which predates the mechanism.

65fd2b9e35 feat(auth): revoke OAuth tokens at the authorization server on clear (#2144)
Clearing the Inspector's OAuth state deleted the local copy of the tokens
and stopped there. The access token — and the refresh token, which is
long-lived by design — stayed valid at the authorization server until they
expired on their own, so a day of connect/disconnect iteration left it
holding grants for sessions that ended hours ago. RFC 7009 §1 describes
exactly that case, and nothing in the repo constructed a revocation
request, so a server author implementing `revocation_endpoint` had no way
to exercise it with the tool that exists to exercise this.

core/auth/revocation.ts builds and sends the request; the three clear paths
call it, so the behavior is written once — web's "Clear OAuth state and
disconnect" (both the active and stored-only paths), the TUI's Auth tab,
and the CLI's --relogin. It runs BEFORE the local clear, because the token,
the client credentials and the discovered `revocation_endpoint` all live in
the store the clear is about to empty. The request names the refresh token
when there is one: §2.1 asks the AS to also invalidate the access tokens
issued under the same grant, so one request covers both halves.

Best-effort by construction. Every path returns a TokenRevocationOutcome
rather than throwing: no advertised endpoint means nothing is sent and that
authorization server behaves exactly as before, and a network error, a
non-2xx, a 5s timeout or an unreadable store is reported and nothing more.
Forgetting the tokens is what the user asked for. `lost_authorization_state`
recovery deliberately skips it — it clears a half-finished flow to retry it,
and an authorization that never completed has no grant to revoke.

Client authentication resolves the preregistered slot first, mirroring
BaseOAuthClientProvider.clientInformation; reading only the dynamic slot
would send no authentication for a server configured with `oauth.clientId`.
The Basic credential is byte-identical to the SDK's applyBasicAuth so this
request and the token request cannot present the same secret differently.

Opt out per server with `oauth.revokeOnClear` (default on, only `false`
written to disk) or per CLI run with --no-revoke. Read at clear time rather
than connect time, so toggling needs no reconnect. Turning it off is a
testing affordance: a client that walks away still holding live tokens is a
case a server author may want to reproduce.

test-servers now advertises `revocation_endpoint` and serves POST
/oauth/revoke, faithful on §2.2 (an unknown token is a success) and §2.1
(revoking a refresh token invalidates the access tokens under the same
grant), with oauth-revocation-http.json / oauth-no-revocation-http.json as
showcase configs.

7ca753bc4c fix(auth): accept RFC 8414 metadata served at the OIDC well-known path
The SDK's `discoverAuthorizationServerMetadata` picks its validation schema
from the well-known filename that resolved rather than from the document that
came back, so anything found at `/.well-known/openid-configuration` is
validated as an OpenID provider document — requiring `jwks_uri`,
`subject_types_supported` and `id_token_signing_alg_values_supported`, three
fields RFC 8414 does not define. RFC 8414 §5 permits that filename for general
OAuth metadata, so a conforming plain OAuth 2.0 authorization server is
rejected; and because the parse throws instead of continuing the candidate
loop, discovery aborts and the connection fails outright.

Filed upstream as modelcontextprotocol/typescript-sdk#2733.

`core/auth/oidcDiscoveryCompat.ts` works around it without fabricating a
field: on a failed RFC 8414 candidate it probes the OIDC candidates the SDK
would try next and, when one returns a document that satisfies
`OAuthMetadataSchema` but fails the OIDC schema, serves that body as the RFC
8414 response so the SDK picks the schema that describes it. A genuine OpenID
provider document is left to the SDK's own OIDC leg. Issuer validation runs
unchanged, since the substituted document is the one the server published.

It wraps `effectiveAuthFetch` — above the fetch tracker, the opposite of
`withOAuthEndpointOverrides` — so the Network tab still records the real 404
and the real probe rather than the substitution.

The candidate derivation mirrors the SDK's `buildDiscoveryUrls`, which a fetch
wrapper cannot call, and a test pins it against that export so the two cannot
drift.

Adds `oauth.asMetadataPath` to the composable test server (mirroring
`oauth.resourceMetadataPath`) and the `oauth-rfc8414-at-oidc-path-http.json`
showcase config that reproduces the failure.

9136ab6268 fix(test-servers): describe and test after-first's resource-subscription counting
Copilot review on #2176. All three findings were the same gap seen from
three angles: `"after-first"` counts *resource-subscription* listens, not
listens, and both the README and the option's own doc comment said
"the first listen".

That distinction is load-bearing rather than incidental — the Inspector
opens a listen at connect time whenever a list-change opt-in is live
(#1920), so counting listens would spend the allowance there and refuse
the user's very first Subscribe, leaving the documented repro describing
a trace nobody sees. It is also what a reader of the option would
otherwise get wrong.

- README: say "the first listen that subscribes to a resource" in both
places, and name the connect-time listen as the reason.
- `ServerConfig.modern.neverAcknowledgeSubscriptions`: document the
list-change exemption and contrast it with `true`.
- Integration test for the mode itself, with every list-change opt-in on
so the connect-time listen really is opened: it must be acknowledged
without consuming the allowance, the first resource subscription
acknowledged, the next refused with the stream left `active`.

8535ae0c2f fix(core): say why a never-acknowledged subscriptions/listen ended
A modern-era `subscriptions/listen` is a long-lived request: the first
message on its stream must be `notifications/subscriptions/acknowledged`,
and the JSON-RPC `result` for the listen id is reserved as the
graceful-closure marker. A server that answers the listen request with a
bare `result` is saying "acknowledged and closed in the same breath" — the
SDK settles the subscription `graceful` and rejects the pending `listen()`.

The Inspector read that rejection as an ordinary drop: eight re-lists over
roughly a minute, the badge flickering `Reconnecting…`, and a final bare
`Stream ended` that said nothing about why. Accepting the message was
never wrong — it is a well-formed graceful-closure response. The silence
was.

- `core/mcp/subscriptionAck.ts` owns the predicate. The SDK gives this
close no code of its own — it is the same `SdkError(ConnectionClosed)`
any pre-ack close carries — so the predicate reads the message, since
the alternative is refusing to retry a genuinely transient drop.
- The condition is deterministic, so the stream ends on the first
occurrence instead of spending the whole backoff run. Both routes are
covered: the one a subscribe/unsubscribe reaches, and the one a
reconnect reaches when an acknowledged stream drops and the server has
  since started refusing.
- A `"never-acknowledged"` status of its own, not `"ended"` — `ended`
means an expected close. It surfaces as the thrown error's message, an
orange "Not acknowledged" badge, and a notice in the panel.
- `subscriptions-never-acknowledged-http.json` reproduces it. It
acknowledges the first listen that subscribes to a resource and refuses
every one after, which is what makes the badge reachable by hand.

f62410a9b3 chore(web): connected-flow smoke for the Tools, Resources and Prompts tabs
The web client had exactly one connected-flow smoke and it was the Apps
tab. `smoke:web:browser` proves the bundle boots and paints but never
connects; `smoke:web:app` / `smoke:web:elicit` connect but only drive the
Apps surface. So the three highest-traffic tabs were unexercised through
the prod bundle end to end — everything covering them (unit tests,
Storybook, the integration project) mocks or bypasses the prod server, so
a regression living only in the built bundle or in the prod Hono
backend's wiring of a tab passed all of them.

`scripts/smoke-web-tabs.mjs` drives all three against one server on one
browser launch (~5s added wall-clock): Tools runs `list_items` and
asserts its `structuredContent` section rendered (#1908), Resources reads
`events` with the template list asserted separately — `resources/list`
and `resources/templates/list` are different calls and either can fail
alone — and Prompts fetches the argument-less `simple_prompt`.

Every assertion is on a `data-*` attribute, never on copy, mirroring the
`data-app-status` precedent: each screen now carries a readiness contract
(`data-tool-count` / `data-call-status` and its Resources / Prompts
equivalents), tabulated in the web README and pinned by screen unit tests
so a rename fails loudly there rather than as an opaque timeout here.

The deep-link connect step is extracted to `lib/deep-link-connect.mjs`
rather than copied a third time. That matters because `parseDeepLink`
ignores a link it cannot validate rather than reporting one, so a drifted
copy fails as an opaque connect timeout in whichever smoke was not
updated. `smoke:web:elicit`'s hand-rolled copy is retired onto it.

Chromium-only and deliberately not in `ENGINE_SMOKES`: the engine tiers
exist for the MCP Apps sandbox's genuinely divergent primitives, and
these tabs are ordinary React and Mantine. Network, Protocol, Logs and
Tasks remain uncovered — a stated gap, not an oversight.

5933162dd0 fix: render tool schemas with root-level anyOf/oneOf composition
A tool's `inputSchema` may carry composition keywords at its root — the
2026-07-28 revision makes this explicit — but both form builders enumerate
the root's `properties` and nothing else. A schema keeping its fields on
`allOf`/`oneOf`/`anyOf` branches therefore rendered no controls at all: no
branch picker, no fields, not even the raw-JSON fallback a union-typed
*property* falls back to, so the tool could only be called with empty
arguments.

Adds `core/json/rootUnion.ts`, one flattening shared by all three clients so
they cannot drift on which schemas they can render:

- `allOf` is merged unconditionally (conjunctive — there is no choice to
present), a root `oneOf`/`anyOf` is returned as branches.
- Web `SchemaForm` renders a Variant picker above the fields and prunes the
outgoing branch's values on a switch, since they are no longer visible and
would describe a shape the call is not making.
- TUI `schemaToForm` gives each branch its own section with optional fields —
ink-form is static, and only one alternative applies to a call.
- `convertToolParameters` looks through the branches, so a CLI `--tool-arg`
is typed by the schema that declares it rather than sent as a string.

Required-field gating asks whether *any* branch is satisfied, which is
selection-independent and never blocks arguments the schema accepts. A
`const` is seeded like a `default` and rendered read-only: it admits exactly
one value, so anything else the user could type produces a rejected call.

Adds a `root-union-schemas-http.json` showcase server.

2cb23a8b21 fix: map the new exit code, keep add runnable, distinguish severity by shape
Copilot review round 4.

- Add `SCHEMA_UNPORTABLE` to `codeForExit`. `CliExitCodeError` lets a
caller omit the envelope, so without the case an envelope-less throw
reported the generic `error` — the same silent mislabelling the rename
was meant to prevent, one layer down. Covered via the envelope-less
  construction that exposed it.

- Keep a local `type` (and the preset's `required`) alongside the remote
`$ref` in the showcase config. The TUI's `schemaToForm` dispatches on
`type`, so a `$ref`-only property fell through to a string field that
`add`'s numeric handler then rejected — making the README's "every
showcase tool is runnable" claim false there. The `remote-ref` finding
is unaffected, since that rule reads `$ref` independently.

- Restrict `type-union` to well-formed unions: non-empty, unique, and
all recognized JSON Schema type names. Its message asserts the
construct is *legal* JSON Schema, which is false for `[]`, `[3]` or
`["bananas"]`, and the `anyOf` it suggested for those would be invalid
too. Malformed schemas stay the SDK parser's business.

- Drop `if`/`then`/`else` from the presence-only allowlist. They only
constrain as a pair, so `{"if": {"const": 1}}` accepts every value and
was evading `untyped-schema`; the pairing is now checked explicitly.

- Distinguish the web sidebar severities by SHAPE, not just colour — a
circle for an error, a triangle for a warning — and name the severity
in the tooltip. Colour alone left a colour-blind reader unable to tell
  them apart.

- Fix the stale "across 2 tools" count in the CLI README's hint sample.

c389565783 fix: address Copilot review on the schema portability lint
- Scope `non-object-root` to `inputSchema`. The pinned SDK types
  `inputSchema` with `type: literal("object")` but `outputSchema` as a
bare `looseObject`, so a `{"type":"string"}` output is conforming; the
rule was claiming a requirement MCP does not make, at error severity.

- Decide `untyped-schema` from an allowlist of constraining keywords
rather than a denylist of annotations. JSON Schema ignores keywords it
does not recognize, so `{"vendorHint": true}` accepts every value and a
denylist passed it as constrained. `$defs`-only schemas now flag too.

- Walk `dependentSchemas` and draft-07 `dependencies`, with a test
pinning that the property-name-array form is not a schema position.

- Suggest `anyOf` branches for an array-form `type`, never "drop it from
`required`" — accepting `null` and permitting absence are independent
contracts, so the old suggestion silently changed the schema's meaning.

- Reject `--strict` alongside `--app-info`. That path returns NDJSON from
`runMethod` and never reaches the lint, so the flag was a no-op gate
  that could never fail.

- Colour the web sidebar flag from the `--inspector-*` severity tokens,
matching the detail panel and avoiding the default yellow that failed
  the a11y check.

- Add a live integration test for `rawToolSchemas`. It caught that an
`outputSchema` override on a preset returning no structured content
makes every call to that tool fail client-side validation — the
showcase config did exactly that, so the flagged tool could not be
run. Moved the bare `true` onto `get_temp`, documented the caveat, and
  pinned both halves.

- Drop the double cast from the lint test fixture.

2f9e0901c1 feat: flag unportable tool schemas in all three clients
Closes #1005

A tool schema can be perfectly legal JSON Schema and still be rejected by
the client the server is meant to run against — Go's `jsonschema` emits a
bare `true` for `interface{}`, which the SDK accepts and Claude Code
refuses with an opaque "Invalid input". The Inspector passed it through
silently.

Adds `core/json/schemaLint.ts`, a portability lint over a tool's
inputSchema/outputSchema, and surfaces one shared verdict everywhere:
the CLI's `--strict` report (exit 6 on an error-severity finding), the
TUI's tool list marker + detail pane, and the web Tools sidebar flag +
detail section.

Deliberately not a JSON Schema validator: a census of 617 public servers
found zero that fail the SDK's own parse, so a conformance check would
report nothing on real servers. Each rule is instead a construct that is
legal JSON Schema and known to be refused or mishandled by a shipping
client — a bare boolean in a schema position, an array-form `type`, a
remote `$ref`, a schema with no validation keyword, a non-object root.

`test-servers/configs/unportable-schemas-http.json` exercises every rule,
via a new `rawToolSchemas` override — the Zod-built presets cannot emit
any of these shapes, which is the same reason a real server only hits
this when its schemas come from another generator.

8d193461e3 fix(scripts): pin the tsbuildinfo inside build/, assert on cache hits
Two findings from review round 1, both real.

The default incremental cache path derives from `rootDir`, so it landed at
`test-servers/tsconfig.tsbuildinfo` — beside the config, OUTSIDE `build/`.
That survives the `rm -rf test-servers/build` this project documents as the
remedy for a deleted source file, leaving tsc convinced its outputs are up
to date and emitting nothing. Pinned to `./build/tsconfig.tsbuildinfo`, and
verified that removing the output dir now forces a full re-emit.

The build is memoized per repo root; the existence assertion was memoized
with it. A later caller naming an entry the first did not would be handed a
path tsc never emitted, then fail with exactly the opaque
ERR_MODULE_NOT_FOUND the helper exists to replace. The assertion now runs on
every call.

e59452ea8c chore(scripts): build test-servers unconditionally from one shared helper
Five `scripts/*.mjs` consumers each carried their own `ensureTestServer()`
that returned early when `test-servers/build` was already on disk. Presence
is not freshness: once that directory exists — which it does on any machine
that has run the smokes once — an edit to `test-servers/src` was never
picked up again, and the smoke went on driving the previously-built fixture.

The failure is silent and misattributed. It does not read as "your fixture
is stale"; it reads as a product bug in whatever the smoke is checking.

`scripts/lib/ensure-test-servers.mjs` is now the one place that build is
produced for a script consumer, and it builds unconditionally — memoized
once per process per repo root, so pack-and-verify's two asks stay one tsc
pass while a fresh run always rebuilds. Callers name the entries they
depend on and get one actionable failure message, replacing five that had
drifted. pack-and-verify also stops shelling out to `npx tsc`, which is
what made it the one script unrunnable on Windows (#1939).

`test-servers/tsconfig.json` gains `incremental` so the up-to-date case is
cheap: 3.0s cold, 1.6s incremental here.

Unconditional emit is still not a clean — `--noCheck` leaves the stale .js
of a deleted src file behind — which is unchanged from before and recorded
in the helper's header and AGENTS.md.

597e1bd25f fix(auth): carry resource_metadata into first-time legacy authorization
The challenge-driven plumbing did not reach the default CLI/TUI flow. With
no stored token in the legacy era, InspectorClient deliberately builds the
transport with no authProvider (so the SDK cannot open a browser before the
callback server is listening), which also leaves challenge interception off;
the 401 then surfaces as the SDK's headerless UnauthorizedError and the
runner calls authenticate() with no challenge in hand. So a server serving
metadata only at the advertised path still failed there — including for the
legacy showcase the README instructs.

Add a passive counterpart to the challenge interceptor: an observer fetch
that reports every 401/403 without touching control flow or the body. The
OAuthManager records the advertised URL from it and uses it in
authenticate() and its CIMD pre-registration. Latest-wins, including
clearing when a later challenge advertises none.

Also: the fixture helper dropped an explicitly supplied empty
resourceMetadataPath by truthiness, bypassing validation and silently
reverting to the well-known route.

Covered by a direct-client integration test that drives connect ->
authenticate() against the real server and asserts the advertised path was
requested and the well-known one never was; verified failing without the
fix.

8beba25b58 fix(auth): address review — reject a query or fragment in the metadata path
`isOriginRelativePath` compared the resolved href against the literal, which
`URL` preserves a query and fragment in — so `/doc?version=1` and `/doc#section`
passed. Express matches on the path alone (and treats `?` as a pattern
character), and a fragment never reaches the server, so the advertised URL
could not resolve to the registered route. Reject both, with cases added to
the table-driven test.

2a0290df26 fix(auth): address review — proxy CIMD discovery fetch, validate metadata path
Two findings from review:

- `ensureCimdClientRegistration` ran protected-resource discovery on the
global fetch while its AS-metadata leg used the configured one. On web
that is `createRemoteFetch`, which proxies through the backend to sidestep
CORS — so the discovery this PR routes to a challenge-advertised URL would
fail in the browser, be swallowed by the catch, and leave CIMD probing the
wrong authorization server. Forward `params.fetchFn`, and assert in the
test that nothing reaches the global.

- `oauth.resourceMetadataPath` accepted `//other-host/doc`. Resolved against
the request base that re-points the origin in the advertised
`resource_metadata` while Express still registers the route locally, so the
server would advertise a document it does not serve. `/\\host` does the
same, since the URL parser folds a backslash into a slash for special
  schemes — a leading-slash check misses both. Validate origin-relativity
with a shared predicate, used by the JSON-config validator and again at
server setup for a programmatically built ServerConfig.

f8a46726b7 fix(auth): pass the challenge's resource_metadata URL to SDK auth()
An RFC 9728 `WWW-Authenticate: Bearer resource_metadata="…"` challenge was
parsed and then dropped by the normalized `AuthChallenge`, so every
challenge-driven `mcpAuth()` call ran discovery without it and probed
locations derived from the MCP server URL instead. OAuth failed outright
against any server whose metadata document lives off the well-known path.

The SDK handles this correctly inside its own transports, but the Inspector
never reaches that path — it intercepts the 401 and drives `auth()` itself
to capture the authorization URL and run the step-up scope union.

Carry the advertised URL on `AuthChallenge` as a string (it crosses the web
client's remote-backend boundary as JSON), convert it at the OAuth boundary,
and pass it to the standard challenge leg, the forced step-up leg, and CIMD
pre-registration — which runs before `auth()` and did its own discovery. A
malformed value is ignored rather than surfaced, matching the SDK's parser.
The callback leg needs nothing: SDK `auth()` persists the URL in its
discovery state.

Adds an `oauth.resourceMetadataPath` test-server mode that moves the
document off the well-known path entirely, so the integration test proves
discovery genuinely fails without the fix.

test(web): prove the dedicated app origin end to end
`frame-ancestors` is checked against every ancestor, not just the parent.
A published app document is framed by the sandbox proxy, which is framed
by the Inspector page — so admitting only the proxy's origin blocked the
frame outright (it rendered as a chrome-error frame that never reached
the bridge). `appDocumentEmbedders` now admits both.

That failure was invisible to every unit test, so `smoke:web:app` grows a
second phase: the same demo app declaring `_meta.ui.domain`, driven
through the same connect → open → ready chain, then asserting the two
facts that only hold on the dedicated path — the inner frame was
navigated to a published document, and its own `location.origin` is a
real http origin rather than "null". Both phases end at
`data-app-status="ready"`, so nothing short of reading the frame's origin
can tell them apart.

Adds `test-servers/configs/mcp-app-domain-http.json` (and a `domain`
param on the `mcp_app_demo_widget` preset) as the showcase server for
both the smoke and hand testing.

f3878f94a9 fix: widen the test recorder's metadata type and correct a stale comment
`RecordedRequest.metadata` was still `Record<string, string>`, so the recorder
advertised a string-only contract it no longer honors. The new CLI tests assert
objects and arrays against it and passed only because `toEqual` does not
enforce the declared type.

Typed as `Record<string, unknown>` rather than core's `RequestMetadata`:
test-servers compiles standalone (`tsc -p test-servers`, NodeNext, no `paths`),
so it has no `@inspector/core` alias to import through.

Also update `progressTokenOf`'s doc comment, which still claimed an invalid
token "reaches the wire inside `_meta`" — untrue since mergeMeta started
deleting it. It now names the split: progressTokenOf governs callback
correlation, mergeMeta governs wire sanitization, both via isProgressToken.

44010c313b test(apps): cover the modern MRTR leg of app-rendered elicitations
Both review findings from round two.

The routing change sits in the funnel BOTH entry points share, but only the
legacy inbound handler was driven by a test — the modern leg, where an
elicitation arrives as an `input_required` result the MRTR driver unpacks and
retries, was covered by construction alone. Adds
`mrtr_app_choose_option` (a modern MRTR tool whose *embedded* elicitation
carries `_meta.ui.resourceUri`) and drives it live over a real transport: the
app answers, the retry carries that answer through `inputResponses`, and the
server echoes it back as the tool's result. A second case cancels the tool
call while the app is up and asserts the request-scoped signal aborts; a third
runs the same tool against a modern server that never advertised the
capability and asserts the native queue takes it.

The cancellation case turned up a real gap: `tryAppElicitation` mounted an app
for an already-aborted signal (the caller cancelled during an earlier MRTR
round), leaving a modal nobody was waiting on and a promise that could never
settle. It now throws the abort straight through.

Also fixes the smoke count in the root README, which still said "two"
headless-Chromium smokes and then introduced a third.

08fc20a22d feat(apps): negotiated app-rendered form elicitations
Let a server hand a form `elicitation/create` to an MCP App and return the
app's ordinary `ElicitResult`, with the native elicitation form as the
fallback on every failure.

No second extension, no custom method, and no custom result shape: the only
new wire surface is a nested `elicitation` flag on the existing
`io.modelcontextprotocol/ui` extension on each side, plus
`_meta.ui.resourceUri` on the request. App rendering is selected only when
all four gates hold — client `elicitation.form`, client MCP Apps MIME type,
the nested `elicitation` setting on BOTH peers, and a valid absolute `ui://`
URI on the request.

Only the web client advertises the nested client-side setting, and only
because it has a sandbox renderer: supplying
`InspectorClientOptions.appElicitation` is what opts a client in, so CLI and
TUI keep advertising the MIME type without ever claiming they can resolve an
elicitation through an app.

Routing lives in the one funnel both entry points already use
(`enqueuePendingElicitation`), so the inbound handler and the MRTR driver
cannot diverge. Ownership is request-scoped — a per-request id keys the
renderer, iframe and bridge — so two concurrent elicitations can never
resolve through each other's bridges. An explicit `decline`/`cancel` is a
completed elicitation and goes back to the server; everything else (absent or
malformed metadata, resource/sandbox/bridge failure, an app with no
elicitation capability, a timeout, an invalid result or one that fails the
requested schema) falls back.

The Inspector speaks the ext-apps#733 / SEP-3118 wire protocol but cannot yet
consume its helpers — the released `@modelcontextprotocol/ext-apps` (1.7.5)
predates that PR. `core/mcp/appElicitation.ts` and
`AppRenderer/requestAppElicitation.ts` mirror it exactly and are marked for
deletion once a release containing it ships. One consequence needed its own
seam: ext-apps 1.7.5 parses the view's `ui/initialize` through a schema that
strips `elicitation`, so an app that correctly advertises it would look like
one that did not, silently turning every negotiated elicitation into a
fallback. `AppRenderer/appCapabilities.ts` records the raw frame instead, and
prefers the bridge's own value once it carries the key.

`AppRenderer` now takes an `AppRenderSource` union rather than a `Tool`, so
the same renderer and bridge factory serve an App tool and an elicitation
without either faking the other's shape.

Adds the public fixture (`app_choose_option` + a self-contained
`ui://demo/choose-option.html` app covering accept/decline/cancel), its two
showcase configs, and `smoke:web:elicit`, which drives the negotiated path
end to end in headless Chromium and then the same tool against a server that
never advertised the capability, asserting the native form takes it.

1c24eda429 fix: validate the whole varspec, and derive the TUI's message from its gate
Addresses Copilot's round-7 review on #2035. Two suppressed comments, both
real; each reproduced against this branch before acting.

1. `parseVarSpec` was a "strip the first `*`, trim, split on `:`" pass, so
it accepted shapes RFC 6570's varspec production does not admit --
   measured on the previous commit:

x://{*id} name "id" => "x://abcdef" explode is a TRAILING modifier
x://{id*:3} name "id" => "x://abc" the two modifiers are exclusive
     x://{ id }    name "id"  =>  "x://abcdef"   whitespace is not a varchar
     x://{a b}     name "a b" =>  "x://1"        ditto -- and it emits into the URI

`{id*:3}` is the worst of them: it silently truncated the value through a
combination the grammar forbids. Parsing is now one anchored regex --
a varname, then at most one trailing `*` or `:1-9999`.

The name set is `varchar` widened by `-` and `~`, deliberately. A name is
the one thing expansion emits into the URI WITHOUT encoding it (`;`, `?`
and `&` write `name=value`), so the rule it has to satisfy is "needs no
encoding" -- RFC 3986 unreserved plus pct-encoded, of which the RFC's own
varchar is a subset. Strict varchar would refuse `{user-id}`, which real
servers publish and which the SDK's matcher round-trips; refusing to read
a resource that demonstrably works is a worse outcome for a debugging
tool than a name one character outside the production. Everything now
rejected is a shape we would otherwise have to guess at.

2. The TUI built its "Missing required template variable(s): ..." list with
its own bare `values[name]` filter while gating on `hasRequiredValues`,
which reads own properties only. For a variable legitimately named
`constructor` the two disagreed: `({})["constructor"]` is `Object`, whose
`.length` is 1, so the list came out empty and the error named no field
at all while the submit stayed blocked. Both now come from one exported
`unmetRequiredGroups`, with `hasRequiredValues` defined in terms of it.

Also renames a test whose title still said multi-name query expressions were
left "to the SDK, which handles them" -- expansion has been owned here since
935113d1; the behavior it asserts is naming, not delegation.

npm run ci passes.

409dce37be fix(core): share URI-template expansion and correct the SDK's multi-name branch
Addresses Copilot's review on #2035. All three findings reproduced against
the pinned SDK before acting.

1. `#` was misclassified as required. Measured: `x://a{#frag}` with no
   `frag` expands to exactly `x://a`, a well-formed URI naming a real
   resource -- unlike `{+path}` (`x://a/`) or a simple `{userId}`
(`file:///users//profile`), which leave an empty path segment. Moved `#`
into the omittable set; the required cases now assert what the URI
*becomes* when blank, so the rule is checked rather than asserted.

2. Delegating to the SDK did not actually give RFC 6570 expansion.
   `UriTemplate.expandPart` takes an early `names.length > 1` branch that
raw-joins values, skipping both `encodeValue` and the operator prefix:
   `x://{a,b}` with `a = "foo/bar"` expands to `x://foo/bar,q` -- the very
unencoded-slash defect this PR is about -- and `x://a{/p,q}` to
`x://ax y,z`. Only `?`/`&` are correct, being dispatched earlier.

Fixing that in the web client alone would have left the TUI and CLI
wrong, since `readResourceFromTemplate` expands through the same class.
So parse/classify/expand now live in `core/mcp/uriTemplate.ts` and both
call sites use it. The correction is surgical: a multi-name non-query
expression is expanded here and spliced in as literal text before the
SDK sees the template (safe -- both encoders escape `{`/`}`), while
every single-name and query expression still goes through the SDK
untouched. The preview applies the same correction, so it cannot
promise a URI that submitting would not send.

3. The showcase promised a blank `{?topic}` read that did not work.
   `UriTemplate.match()` compiles `{?topic}` to a *required*
`\?topic=([^&]+)`, so `match("foobar://events")` returns null and the
read 404s. A real server exposes the unfiltered collection as its own
resource; the showcase now registers `foobar://events` so the
documented step resolves. Verified end to end through the CLI.

83658db2dd fix(web): expand resource templates per RFC 6570
The Resources screen discovered and substituted template variables with a
bare `/\{(\w+)\}/g` regex, which is wrong two ways: it cannot see an
expression carrying an operator, so `foobar://events{?topic}` rendered no
input at all; and it splices the raw value in, so a `/`, `?`, `#`, `%`,
space or non-ASCII character in a simple `{topic}` landed unencoded and
changed the URI's structure -- `foo/bar` produced an extra path segment
that a conforming matcher rejects with `-32602 Resource not found`.

Delegate expansion to the SDK's `UriTemplate`, the same implementation
`InspectorClient.readResourceFromTemplate` (and so the TUI) already
expands through, so the two clients cannot disagree about what a template
means. The new `utils/uriTemplate` supplies only what that class does
not: which variables to render an input for, which of them a read cannot
proceed without, and a partially-expanded preview.

Required-ness follows the operator. Under `?`, `&`, `.` or `/` the whole
expression is omitted when the variable is undefined, so those fields are
marked Optional and reading with them blank is a legitimate request for
the unfiltered resource; under `""`, `+` or `#` the variable sits mid-URI,
so it stays required. Blank fields are dropped before expanding so an
untouched optional field reads as undefined rather than as the empty
string, which would expand to a valueless `?topic=`.

Adds the `rfc6570_templates` preset and a `rfc6570-templates-http.json`
showcase server serving the two templates from the issue.

b3aedf62cd fix(web): name an empty tool result instead of "No results yet" (#1860)
The Tools screen mounts the Results panel only once a call has produced a
result, but the panel's zero-content branch rendered "No results yet" — the
same wording it would use before anything had run. An empty `content` array
with no `structuredContent` is a legal `CallToolResult`, so a call that had
plainly completed reported itself as a call that never happened.

That reads worst after a multi-round-trip (MRTR) sequence, which is how it was
reported: the user answers one or more embedded elicitations, watches the
Protocol tab group the exchange and mark it COMPLETE, and is then told there
are no results yet. Nothing was lost — the result was stored and rendered; the
panel simply had no wording for "completed, returned nothing".

The panel now renders an "Empty result" alert saying the call completed and
returned no content. This is the remaining half of the same gap #1908 closed
for `structuredContent`-only results.

test-servers: new `mrtr_empty` preset (`createMrtrEmptyResultTool`) — one
elicitation round, then a complete result with `content: []` and no
`structuredContent` — added to `mrtr-showcase-http.json` so the shape is
reproducible by hand. README documents it.

937f1dd51b fix: render nullable enum fields as a Select instead of a raw JSON textarea
A tool argument that is optional AND explicitly nullable — Zod's
`.nullish()` / `.nullable().optional()` — compiles to
`anyOf: [{ type: "string", enum: [...] }, { type: "null" }]`. Every branch
of the tool-call form's field dispatcher tests a single `type` string, so
such a field matched nothing and fell through to the raw-JSON fallback,
whose onChange stores the unparseable in-progress text back into state and
re-`JSON.stringify`s it on the next render — compounding a fresh layer of
escaping on every keystroke.

`normalizeUnionType` already existed for the FastMCP `string|null` shape,
but it only copied the surviving branch's `type` and dropped everything
else, so the `enum` never reached the dispatcher; and nothing called it on
a tool input schema — only elicitation requests, via
`resolveRefsInMessage`. So:

- Rewrite `normalizeUnionType` to hoist the non-null branch's own keywords
  (`enum`, `items`, `properties`, `minimum`, …) onto the flattened schema,
as v1.x did. This also collapses nine near-identical blocks into two and
picks up the `object`/`array` cases the `type: [X, "null"]` form was
  missing.
- Normalize each field in `SchemaForm.renderField`, so tool, elicitation,
and app forms all get it rather than just elicitation.
- Give a nullable enum `Select` a clear affordance, so a value can be set
back to `null`; a non-nullable enum keeps none, since it would reject it.

Adds a `record_shipment` preset and `nullable-fields-http.json` showcase
config serving all four nullable scalar shapes, for manual verification.

6f47eaa143 fix: render a tool result's structuredContent in the Tools screen (#1908)
A tool declaring an `outputSchema` returns its real payload in
`structuredContent`; the `content[]` blocks usually only summarize it. v1
rendered that payload as its own inspectable JSON section — v2's
ToolResultPanel only ever walked `result.content`, so the payload was
dropped with no label, section, or hint that it existed.

Adds a `StructuredOutputPanel` group: a collapsible "Structured Output"
box (bordered like the existing "Resource Links" box) rendering the
payload as pretty-printed, syntax-highlighted, copyable JSON, bounded so
a large payload scrolls within itself. ToolResultPanel renders it below
the content blocks — and also when `content` is empty (where "No results
yet" was simply wrong) and below the alert on an error result, so it is
never silently dropped.

Adds a `list_items` test-server preset (nested payload, the issue's repro
shape) and a `structured-output-http.json` showcase config, documented in
the README.

4340283f70 test: add a duplicate-tool-names test server for the #1957 repro
Nothing in the repo could serve the shape this fix is about: every preset
registers a unique name, and the SDK's `registerTool` rejects a repeat. So the
only reproduction was a unit test — there was no way to smoke the real app
against a server that returns duplicates.

Adds `ServerConfig.duplicateToolNames` (a list of registered tool names) to the
composable test server, plumbed through the JSON config, and a showcase config
`duplicate-tool-names-http.json`. The named tools are emitted a second time in
`tools/list` with the same `name` and a "(duplicate)" title.

The repeats are appended after the whole list rather than placed beside their
twin. That is both the realistic shape (two tool sources concatenated) and what
makes the defect observable: React matches a leading run of same-key children
first, so a head-adjacent duplicate happens to line up and the bug hides. Only
a separated pair collides in the keyed map and orphans a row — verified both
ways against the prod bundle.

The `tools/list` override is shared with the existing pagination handler (which
already hand-builds the list); with no `maxPageSize` it returns a single
unbounded page. Duplication happens before pagination, so a duplicated pair can
straddle a page boundary the way a real server's would.

Also adds a ToolControls test for this appended ordering, distinct from the
reporter's interleaved fixture, and documents the config in the README showcase
table.

28a21c5e08 test: add an MCP App showcase config to reproduce and verify #1859
The `mcp_app_demo` tool and `mcp_app_demo_widget` UI resource presets already
existed in the preset registry, but no config wired them into a server — so
there was no runnable way to reach a rendered MCP App, and therefore no way to
exercise the sandbox proxy path that #1859 broke.

`mcp-app-http.json` composes the two over plain streamable-HTTP. With it, the
Apps tab renders a real widget and the packaging bug is reproducible on demand:
without clients/web/static in the tarball the widget area shows "Sandbox not
loaded: ENOENT … /clients/web/static/sandbox_proxy.html" instead.

chore(deps): migrate to eslint 10 to clear the brace-expansion DoS advisory (#1900)
* chore(deps): migrate to eslint 10 to clear the brace-expansion DoS advisory (#1838)

eslint 9 pulled `minimatch@3` → `brace-expansion@1.1.16`, which carries
GHSA-mh99-v99m-4gvg (high, DoS via unbounded expansion length). It appeared in
all five lockfiles — the root plus each of the four clients, none of which is an
npm workspace, so all five had to move together.

Deferred from the pre-2.0.0 sweep because it is dev-scope, not attacker-
controlled, and a major bump across five packages was the wrong thing to attempt
immediately before an irreversible publish. This is that work.

## What moved

- `eslint` `^9.39.4/5` → `^10.8.0` and `@eslint/js` → `^10.0.1`, in all five
  manifests.
- `eslint-plugin-react-hooks` `^7.0.1` → `^7.1.1` in web and tui. Required, not
cosmetic: 7.0.1's peer range stops at `^9.0.0`, so installing it alongside
  eslint 10 fails `ERESOLVE`.
- Everything else already declared eslint 10 support and stayed put:
  `typescript-eslint@^8.56.1` (`^8.57 || ^9 || ^10`),
  `eslint-plugin-react-refresh@^0.5.2` (`^9 || ^10`),
  `eslint-plugin-storybook@^10.2.19` (`>=8`).

## Config migration

None was needed. All five configs were already flat (`eslint.config.js` with
`defineConfig`/`globalIgnores` from `eslint/config`), which is the format v10
keeps and the only one it still supports. The root config's `core/` and shared-
surface blocks — including the `_`-prefix `argsIgnorePattern` /
`varsIgnorePattern` / `caughtErrorsIgnorePattern` — are unchanged and still gate
`lint:core` and `lint:shared`.

No rule was disabled, downgraded, or scoped away to make the new eslint pass.
v10 surfaced 21 real violations across three rules; all 21 are fixed in the code.

### `preserve-caught-error` (new in `eslint:recommended`) — 15 sites

A `throw new Error(msg)` inside a `catch` silently drops the original error.
Each now passes `{ cause }`, so the underlying failure survives into the chain.
Messages are unchanged, so nothing that asserts on them moves. 12 in `core/`,
2 in `clients/cli`, 1 in `test-servers/src`.

### `no-useless-assignment` (new in `eslint:recommended`) — 2 sites

`clients/cli/src/cli.ts`'s `optionArgs` and `test-servers/src/test-server-oauth`
's `valid` were initialized and then unconditionally overwritten on every path.
Both are now declared without the dead initializer; TypeScript's definite-
assignment analysis covers the remaining paths.

### `react-hooks/set-state-in-effect` — 8 sites (web)

These were false negatives the plugin fixed in 7.1.0, not a rule we newly opted
into: `set-state-in-effect` has been `error` in `flat/recommended` since 7.0.1,
and the web config has always extended it. Every one was the same anti-pattern —
resetting or re-syncing local state from a prop inside a `useEffect`, which
paints the stale value for one frame and then renders again to correct it.

All eight now use a new `useValueChange(value, onChange)` hook
(`clients/web/src/hooks/useValueChange.ts`), which is React's documented
"adjusting state during render": compare against the previous render's value
with `Object.is` and call back *during* render, so React discards the
in-progress output before it reaches the DOM.

- `MrtrConversation`, `ProtocolEntry`, `TaskCard` — mirror `isListExpanded`.
- `NetworkEntry` — same mirror, plus the "Reveal in Network" force-open. The
reveal's rAF scroll stays an effect (real external-system work); only the
`setIsExpanded(true)` moved. `isExpanded` is now seeded from
  `isListExpanded || revealed`, since a render-time sync fires on change and so
cannot cover an entry that mounts already revealed.
- `PromptArgumentsForm`, `ResourceTemplatePanel` — reset on template/prompt
  switch.
- `ServerConfigModal` — reset on open. Keyed on `opened ? initial : undefined`,
which collapses "opened flipped" and "initial changed while open" into one
value; `initial` is already memoized, so it cannot thrash.

`useValueChange` has its own test file, and `ServerConfigModal` gains an
open→edit→close→reopen test (the close leg was previously untested, which is
what left the new guard uncovered).

## Advisory delta

`npm audit`, per package, before → after:

| Package | before | after |
| ------------------- | -------------------------- | -------------------------- |
| root | 9 (1 low, 6 mod, 2 high) | 8 (1 low, 6 mod, 1 high) |
| `clients/web` | 19 (3 low, 6 mod, 6 high, 4 crit) | 17 (3 low, 6 mod, 4 high, 4 crit) |
| `clients/cli` | 6 (2 low, 4 high) | 4 (2 low, 2 high) |
| `clients/tui` | 4 (1 low, 3 high) | 2 (1 low, 1 high) |
| `clients/launcher` | 3 (3 high) | 1 (1 high) |

`brace-expansion` is gone from all five — `eslint@10` resolves `minimatch@10` →
`brace-expansion@5.0.9`. Dropping `@eslint/eslintrc` (removed in v10) also took
`js-yaml` with it, clearing GHSA-52cp-r559-cp3m (high) from all four clients as
a bonus. The remainder are unrelated and tracked separately.

`npm run ci` passes end to end — validate, the per-file ≥90 coverage gate, the
build gate, all five smokes (including `smoke:tui`, run locally on a real TTY),
and 462 Storybook tests.

Also documents the effect rule in AGENTS.md and mirrors it into
`.github/copilot-instructions.md`, since it is now something a reviewer cites.

* docs: state useValueChange's referential-stability requirement (#1838)

Copilot review on #1900: the hook compares with `Object.is`, so an object or
array literal rebuilt in the component body compares unequal every render.
Because the callback is what updates state, that is not merely a wasted render —
it is an infinite loop.

Every current caller passes a primitive (`boolean`, `string`) or an already-
memoized value, so nothing changes behaviorally. But the hazard was implicit,
and the next caller is the one it would bite.

Documented on the hook itself, and one clause added to the AGENTS.md rule with
its mirror in `.github/copilot-instructions.md`. No runtime guard: the
requirement is the same one a `useEffect` dependency array carries, and here the
failure is loud and immediate rather than silent.

`npm run ci` passes.

* fix(cli): stop asserting the --tool-args-json catch binding is an Error (#1838)

Copilot review round 2 on #1900: the `--tool-args-json` parse handler formatted
its message with `(e as Error).message`. A `catch` binding is `unknown`, so that
cast is an assertion rather than a check — a thrown non-Error would render
`--tool-args-json is not valid JSON: undefined`, which points the user at
nothing.

`JSON.parse` does only throw `SyntaxError` today, so this is not a live bug. But
the cast is the kind AGENTS.md asks us not to leave unjustified, the same
handler was already being touched here to add `{ cause: e }`, and every other
site in this PR uses the guarded form. Now it does too.

`npm run ci` passes.

* docs: require useValueChange's onChange to be render-pure (#1838)

Copilot review round 3 on #1900: `onChange` is called during render, so it is
bound by render-phase purity — and the web client mounts under `StrictMode`,
which deliberately double-renders in development. A callback doing anything
external would run an unpredictable number of times, and concurrent React can
abandon an in-progress render entirely.

Every current caller already passes a callback that only calls `setState`, so
nothing changes. But "runs during render" is the whole point of the hook and the
constraint that follows from it was left implicit.

Stated on the hook, with the same clause in the AGENTS.md rule and its
`.github/copilot-instructions.md` mirror. All three now point at `NetworkEntry`
as the worked example of the split: the reveal's force-open is a state update
and lives in `useValueChange`, while its `requestAnimationFrame` scroll stays a
`useEffect`.

`npm run ci` passes.

* chore: close a batch of quality-gate & tidy gaps (#1785 #1787 #1789 #1790 #1767)

Bundles five small, related tidy/gate items into one PR:

- #1785: delete the dead src/types/navigation.ts InspectorTab (0 importers;
  duplicated/drifted from utils/inspectorTabs InspectorTabId). src/types/
now holds only ambient .d.ts stubs.
- #1787: add src/hooks/** to the web coverage `include` (it was silently
outside the ≥90 gate) and add tests for the two untested hooks,
useServerJsonImport and useImportClientConfig, bringing every hook to
  ≥90 on lines/statements/functions/branches.
- #1789: widen clients/{cli,tui,launcher} format/format:check to their
  top-level configs; add root format:shared / format:check:shared covering
  test-servers/src/**, vitest.shared.mts, and the repo-root eslint.config.js.
- #1790: pin prettier to an exact version (3.8.4, dropping the ^3.8.1
caret) in all five package.json files so the format gate's verdict can't
shift with an in-range patch bump; lockfiles synced.
- #1767: its root-file format+lint gap is closed here (format:shared +
  a new lint:shared via a second files block in the root eslint.config.js;
web/server/ was already gated in #1788). Its larger cli/tui __tests__
typecheck piece (76 type errors) is split out to #1791.

Prettier-normalized the newly-covered files (pure formatting). No runtime
behavior change. `npm run ci` green.

* chore: address PR #1792 review findings

- #1789 gap: widen clients/web format to include .storybook and clients/
launcher format to include scripts/; reformat the 3 .storybook files
(were non-conforming under the pinned prettier 3.8.4). Audited all 885
tracked source files — now fully prettier-clean.
- Export VALIDATE_DEBOUNCE_MS / COLLAPSE_DELAY_MS / HIGHLIGHT_DURATION_MS
from useServerJsonImport and use them in the test instead of hardcoded
300/1000/250 (so a tuned constant can't silently desync the test).
- Assert the symmetric renameErrors["alpha"] alongside ["beta"] so a
  one-directional collision-check regression fails.
- Comment the blank-rename test as exercising a UI-unreachable defensive
  branch (canImport gates submit).
- Un-stale the two specification/v2_ux_interfaces_plan.md references to the
  now-removed navigation.ts InspectorTab.

1ce3b64 added .storybook to clients/web's format glob and scripts/ to

Bring it in line and note the non-recursive top-level glob is why nested

* chore: handle PR #1792 round-3 follow-ups in-place (format-coverage guard, uniform globs, theme gate)

Rather than defer these to tickets, fold them into this PR:

- Obs 2: widen every client + root format glob to a uniform
  *.{ts,tsx,mts,cts,js,jsx,mjs,cjs} so a future .mts/.cjs/.tsx top-level
file can't silently slip the gate.
- Obs 1: add scripts/verify-format-coverage.mjs (wired into `validate`) —
it parses the format:check globs out of every package.json, enumerates
all tracked source files, and fails listing any not covered by a glob.
The durable, standing version of #1789's one-shot audit; negative-tested.
- Obs 3: bring src/theme/** under the coverage gate (21/23 files were
already ≥90 transitively; add Paper.test.tsx for the one laggard,
  Paper.ts). src/App.tsx (~4.5k lines, ~42% branches) is converted from a
silent omission into an explicit, documented exclusion (comment on the
  coverage `include` + AGENTS.md) — gating it is a dedicated effort, not a
  whitelist tweak.

npm run ci green (the new guard reports 885/885 tracked source files gated).

Claude-Session: https://claude.ai/code/session_01XNdjEPKLG637X8YmhDiEk5

* chore: address PR #1792 round-4 findings (guard hardening, docs, exclusion list)

- Harden verify-format-coverage: only harvest format:check globs from
scripts transitively reachable from `validate` (via npm run refs), so an
unwired/renamed format:check no longer counts as coverage. Negative-tested
  — renaming clients/web format:check to an unreferenced name now fails the
guard (627 files flagged) instead of passing.
- Name src/main.tsx and src/index.ts (browser + bin bootstraps) alongside
src/App.tsx in the coverage-include comment and AGENTS.md, so the
documented exclusion list is complete rather than claiming App.tsx is the
  only one.
- Doc sync: document verify:format-coverage (AGENTS.md + README script table
  + scripts/ layout line), fix the `validate` ordering (verify:format-
coverage now runs before validate:core) in AGENTS.md and README, and
update the quoted globs to the uniform *.{ts,tsx, mts, cts,js,jsx, mjs, cjs} /
  test-servers *.{ts,tsx,mts,cts}.

npm run ci green.

Claude-Session: https://claude.ai/code/session_01XNdjEPKLG637X8YmhDiEk5

* chore: address PR #1792 round-5 findings (airtight guard + doc/config sync)

- verify-format-coverage: assert the root `validate` chain invokes each
client's `validate` (cd <dir> && npm run validate), closing the last
reachability edge — a client dropped from the root chain now fails the
guard even though its own globs still exist. Negative-tested.
- eslint.config.js: widen the shared `files` block to
test-servers/src/**/*.{ts,tsx,mts,cts} to match the widened lint:shared
glob, so a future .mts/.cts there is lint-gated, not just prettier-gated.
- AGENTS.md: scope the "uniform extension set" claim to client globs
  (core stays {ts,tsx}, shared {ts,tsx,mts,cts}), with verify:format-coverage
as the backstop — removing the self-contradiction with the line below.

npm run ci green.

Refactor / justify codebase-wide 'as unknown as' casts (#1690) (#1756)
* core: refactor serverList.ts object↔Record casts into typed helper (#1690)

Remove the four pre-existing `as unknown as Record<string, unknown>` /
`as unknown as MCPServerConfig` casts in core/mcp/serverList.ts:

- stripInspectorFields: rewrite as clone + `delete` over INSPECTOR_FIELD_KEYS.
Every Inspector-extension key is optional on StoredMCPServer, so the result
is still (a subtype of) MCPServerConfig with no cast.
- oauth-strip in extractSecretsFromStored: `oauth` is optional, so a plain
`delete stripped.oauth` is legal — drop the cast.
- mcpConfigToServerEntries: route the one genuinely-needed widening through a
single documented `toRecord(value: object)` helper. Taking `object` makes the
single `as Record<string, unknown>` legal (no TS2352), so the double cast
  disappears from the call site.

Net: zero `as unknown as` in serverList.ts; the one remaining structural cast
lives inside the audited toRecord helper. Behavior-neutral — existing coverage
(79 serverList tests) unchanged; full `npm run ci` passes.

Closes #1690

Claude-Session: https://claude.ai/code/session_01XNdjEPKLG637X8YmhDiEk5

* core+web+test-servers: eliminate/justify remaining `as unknown as` casts (#1690)

Codebase-wide follow-on to the serverList refactor: audit every `as unknown
as` double cast outside tests and either remove it or ensure it carries a
justifying comment (the review bar is "no *unjustified* double cast").

Removed (widened via a shared helper or narrowed to a single cast):
- Promote the serverList `toRecord(value: object)` widening helper to
core/json/jsonUtils.ts and reuse it at core/mcp/node/config.ts (normalize
loop), core/mcp/remote/node/server.ts (pino level dispatch), and
  clients/web/src/App.tsx (window global lookup).
- inspectorClient.rawWireRequest: type the frame as `JSONRPCRequest` and
narrow only `params` with a single structural cast (the SDK's `_meta`-typed
params is what blocked direct assignment).
- toolOutputValidation: SDK `outputSchema` → `JsonSchemaType` is a single cast.
- test-server-fixtures: the optional `task` handle is reachable with a single
  cast (target field is optional).

Justified in place (genuinely unavoidable — private SDK internals, partial
interface stubs, third-party/nominal bridges): the inspectorClient private-map
accesses, resourceContext + tokenAuthProvider partial `OAuthClientProvider`
stubs, the ext-apps peer bridge, modern/legacy `_requestHandlers` bypasses,
and the happy-dom `matchMedia` test mock.

Behavior-neutral. Full `npm run ci` passes (validate, ≥90 coverage gate,
smoke, Storybook).

* address @claude review nits: consistent cast justifications + drop reflow noise (#1690)

- test-servers/src/modern-tasks.ts: add an inline justification above the
`_requestHandlers` double cast, mirroring the sibling in
composable-test-server.ts (was leaning on the function JSDoc only).
- Annotate the partial-`AppBridge` mock double casts in the story fixtures
(AppsScreen ×2, AppRenderer, InspectorView) so no non-test-file double cast
  is unjustified.
- inspectorClient.ts: revert unrelated Prettier reflow hunks (protocolVersion,
task-content array, timeout-reject wrap, reconnect guard) so the diff is the
  single rawWireRequest cast change only.

Behavior-neutral (comment/format-only). `npm run ci` green apart from a known
flaky redaction test whose assertion collided with a random request id.

* actually revert the reflow noise this time (#1690)

The reflow reverts in cc187ea were silently undone by a `prettier --write` I
ran on the same file right after, so the four Prettier-only hunks from 6a3afa4a
were still in the diff (caught in re-review). Revert them for real now — and
without re-running Prettier on the file:

- core/mcp/inspectorClient.ts: restore the protocolVersion line, task-content
array, timeout-reject call, and reconnect guard to their origin/v2/main form,
so the file's diff is only the single rawWireRequest cast change.
- test-servers/src/modern-tasks.ts: restore the ModernTaskStatus union to its
one-line form (the leading-`|` reflow was also Prettier churn), leaving only
  the new `_requestHandlers` justification comment.

Formatting-only; `npm run validate` green.

Extensions: advertised-extensions toggle in Server Settings (Phase 2 of #1633) (#1743)
* Extensions: advertised-extensions toggle in Server Settings (Phase 2 of #1633)

Adds the debugging knob from #1633: users choose which extensions the
Inspector advertises to a server, and a server may register different tools
in response. Builds on Phase 1's registry + advertisedExtensions option (#1738).

Settings + persistence (core):
- InspectorServerSettings.advertisedExtensions + StoredMCPServer field, wired
  through serverList's read/write converters and INSPECTOR_FIELD_KEY_MAP
  (omit-when-empty, byte-stable round-trip).
- validateSettings (remote PUT route): isBooleanRecord guard + carry a
  non-empty map; unknown/malformed rejected.
- App.tsx threads savedSettings.advertisedExtensions into the client options.

UI:
- ServerSettingsForm renders an "Advertised Extensions" group of checkboxes
driven by ADVERTISABLE_EXTENSIONS; a per-server override wins over the
registry default. Modal folds a toggle into settings.advertisedExtensions.

Test server + proof:
- composable-test-server extensionGatedTools: a tool registered disabled and
enabled on `initialized` iff the client declared the mapped extension
(getClientCapabilities). Legacy stateful leg. Threaded through
  load-config/resolve-config; advertised-extensions-http.json showcase + README.
- Integration test connects legacy with/without the tasks extension advertised
and asserts the gated tool appears/disappears in tools/list — the acceptance
criterion, proven end to end, not asserted.

Tests: serverList round-trip (non-empty/absent/empty), ServerSettingsForm
toggle rows, ServerSettingsModal fold, validateSettings accept/reject, and the
integration gating test. npm run ci green (per-file ≥90 gate held).

* Extensions: add #1739 proof screenshots (advertised-extensions toggle)

End-to-end smoke test in the web client against the legacy
advertised-extensions-http.json test server (port 3220): connecting with the
Tasks extension advertised (default) lists echo + get_weather, unchecking the
Advertised Extensions → Tasks toggle and reconnecting lists only echo — the
server's tool registration changing on the client-declared extension.

Claude-Session: https://claude.ai/code/session_01XNdjEPKLG637X8YmhDiEk5

* Extensions: reconverge advertisedExtensions to no-override on default (review)

Address Phase 2 review observation #1: toggling an extension back to its
registry default now deletes the override key (and drops the whole map to
undefined when it empties), so the on-disk `advertisedExtensions` reconverges
to the byte-stable "no override" state instead of persisting a redundant
`{ key: <default> }`. Matches the omit-when-default policy the other settings
use. Only a value that differs from the default is persisted.

Tools: x-mcp-header annotation tooling + -32602 unknown-tool rendering (#1632) (#1735)
* Tools: x-mcp-header annotation tooling + -32602 unknown-tool rendering (#1632)

Card 9/10 of the SDK-v2 + New Spec stack. Adds SEP-2243 `x-mcp-header`
tooling to the Tools tab and adjusts result rendering for SDK v2's
unknown-tool behavior, and bumps the SDK to 2.0.0-beta.5.

- **`core/json/xMcpHeader.ts`**: a faithful port of the SDK's (non-public)
  `scanXMcpHeaderDeclarations` — validates every SEP-2243 constraint
(primitive types incl. `number`, RFC 9110 token, case-insensitive
uniqueness, statically-reachable `properties`-only chain) and derives the
mirrored `Mcp-Param-{Name}` params for a kept tool.

- **Excluded tools**: the SDK silently drops tools with invalid annotations
from `tools/list`. InspectorClient now re-lists the RAW (unfiltered) list
and scans it to recompute the excluded set with reasons — gated to the
modern non-stdio era (matching the SDK's gate), recomputed on every
aggregate refresh, surfaced via a new `excludedToolsChange` event +
  `getExcludedTools()` and `useInspectorClient().excludedTools`. The Tools
sidebar lists them struck-through under an "Excluded (SEP-2243)" divider
  with the reason on hover.

- **Mirrored args**: ToolDetailPanel shows a "Mirrored request headers"
section listing each arg → `Mcp-Param-{Name}` (with the browser-omits note).

- **-32602 rendering**: SDK v2 rejects an unknown-tool call with `-32602`
instead of an `isError` result, which previously rendered nothing. New
ToolCallErrorPanel renders a thrown tool-call error, with a targeted
  "Unknown Tool" hint for `-32602`.

Adds unit + Storybook + integration coverage (a real modern server with an
invalid-header tool fixture) for every path. `npm run ci`-equivalent checks
green.

* test-servers: x-mcp-header modern showcase config + preset (#1632)

Register the `invalid_header_tool` fixture as a preset and add
`xmcpheader-modern-http.json` — a modern HTTP showcase serving echo, a
valid-header `get_weather`, and the invalid-header tool — so the Tools-tab
x-mcp-header tooling (mirrored headers, excluded-tools surface, -32602
rendering) can be exercised manually. Documented in the README alongside the
other per-SEP showcase configs. Smoke-tested end to end in the web client.

* Respond to review: disambiguate -32602, doc/search nits (#1632)

Addresses the review on PR #1735:

1. -32602 "Unknown Tool" mislabel: `-32602` is generic Invalid params, so a
known tool called with bad arguments hit the same code and rendered under
the definitive "Unknown Tool" heading. Replace `isUnknownToolError` with
   `classifyToolCallError(code, message)` → "unknown-tool" only when the
message names it (not found / unknown tool / …), else "invalid-params"
(new "Invalid Parameters" heading + schema hint), else "generic".

2. Double pagination walk: expand the `listAllTools` comment to document that
the excluded-tools recompute is a deliberately un-cached second raw walk
(doubles list round-trips on modern non-stdio, runs even on a cache-served
aggregate) — accepted for a debugging tool, no-op on legacy/stdio.

3. Doc nit: `scanXMcpHeaderDeclarations` no longer claims to visit "$ref
targets" — reworded to note `$ref` is never followed (refs resolve into the
   unreachable `$defs` body).

4. Search nit: the excluded-list filter now matches name AND title, matching
   the main tools filter.

Claude-Session: https://claude.ai/code/session_01XNdjEPKLG637X8YmhDiEk5

* Tighten unknown-tool matcher to be tool-scoped (#1632 re-review nit)

The re-review noted the -32602 disambiguation was symmetric: broad markers
("does not exist", "unrecognized") could inversely mislabel a genuine
invalid-params message (e.g. "property 'region' does not exist") as "Unknown
Tool". Scope the not-found/does-not-exist family to require the word "tool" in
the same clause (matching the SDK's "Tool <name> not found"); keep "unknown
tool" / "no such tool" as standalone unambiguous markers. Adds a
tool-less-"does not exist" test asserting it classifies as invalid-params.

Claude-Session: https://claude.ai/code/session_01XNdjEPKLG637X8YmhDiEk5

* Add trigger_invalid_params showcase tool + Invalid Parameters screenshot (#1632)

Adds a `trigger_invalid_params` spec-error trigger to the modern showcase
server: the injector answers its `tools/call` with a real `-32602 Invalid
params` JSON-RPC error whose message does NOT name an unknown tool. This lets
the "Invalid Parameters" branch of the tool-call error panel (added in the
review response) be exercised live, distinct from the unknown-tool `-32602`.

- `SPEC_ERROR_TRIGGERS` gains the `trigger_invalid_params` entry (HTTP 200,
  in-band -32602) + preset registration.
- Enable `injectSpecErrors` on `xmcpheader-modern-http.json` and add the tool.
- README + pr-screenshots README document it; adds the live "Invalid
  Parameters" proof screenshot.

* Tasks: modern io.modelcontextprotocol/tasks extension (era fork core)

Core + test-server implementation of the SEP-2663 tasks extension for modern
(2026-07-28) connections, forked from the legacy 2025-11-25 tasks path.

SDK v2 removed all tasks support AND era-gates the tasks/* spec methods out of
the modern era on both the client (outbound) and server (inbound) sides, and its
codec rejects a resultType:"task" result outright. So the Inspector drives the
extension itself: task creation rides tools/call (a modern method) with the
resultType:"task" frame rewritten at the transport to a CallToolResult carrying
the handle in _meta; tasks/get/update/cancel go over a raw-wire request channel
that bypasses the era gate (full modern envelope, string ids, consumed by the
transport). input_required tasks reuse the existing pending-request modal via
fulfilInputRequests, answered with tasks/update. No tasks/list, no tasks/result.

Tab + run-as-task gate on the negotiated extension; the managed task store's
refresh re-polls known handles on modern (no server list). Test server serves
the extension via an Express interceptor (SDK can't) + a shared poll-driven
runtime. Full integration coverage of both eras.

* Tasks modern era: tests + coverage (raw-wire, schemas, era integration)

Adds unit coverage for modernTaskSchemas and the raw-wire request channel
(transport-null guard, send failure, timeout, error response, teardown), plus
end-to-end era integration tests (create/poll/input-required/update/cancel,
synchronous passthrough, unknown-id error, modern store re-poll). Fixes the App

Document the tasks era-fork test configs in README and the new core/test-server

* Tasks modern era: offer Run-as-task for any tool + PR screenshots

On modern (SEP-2663) task creation is server-directed — the client declares the
extension once and any tool may return a task — so the Tools screen now offers
'Run as task' for every tool on a modern connection (threaded via a modernTasks
prop through InspectorView → ToolsScreen → ToolDetailPanel), rather than gating
on the legacy per-tool taskSupport. Verified end-to-end in the web UI (remote
proxy): create → poll tasks/get → input_required elicitation → tasks/update →
inline result, plus the unchanged legacy tasks/list/result flow. Adds proof
screenshots.

* Tasks modern era: address two follow-ups (task-input note + ordinary-path auto-poll)

1. The pending-request note now distinguishes a modern task round from an MRTR
round: a new 'task-input-required' PendingRequestOrigin threads through
fulfilInputRequests → the pending-request modal → MrtrOriginNote, which shows
'submitted via a tasks/update request (SEP-2663), not a retry' instead of the
   MRTR retry copy.

2. The ordinary (non-run-as-task) modern callTool path now auto-polls an
unsolicited task handle: attemptToolCall detects the rewritten handle in the
result _meta and drives it to termination (get → input_required/update →
inline result), dispatching task events so the Tasks tab + toasts track it and
the tool result resolves to the task's final result. The poll loop is shared
with the streaming path via extracted helpers (submitModernTaskInput /
   modernTaskTerminalOutcome / modernTaskPollInterval).

* Tasks modern era: address review (negotiated envelope version, input-round cap, clarifying comment) + fresh smoke screenshots

Review of #1731:
1. withModernTaskEnvelope now stamps the NEGOTIATED protocol version
(getProtocolVersion() ?? MODERN_PROTOCOL_VERSION) so the envelope matches the
MCP-Protocol-Version header instead of a hardcoded constant.
2. A never-completing input_required task is now bounded by MRTR_MAX_ROUNDS in the
shared submitModernTaskInput (threaded through both poll loops), so a
   non-conformant server can't re-prompt the user forever. New modern_loop_task
test-server tool + integration test cover the cap.
3. ToolDetailPanel: comment clarifying that per-tool taskSupport:"forbidden" is
deliberately ignored on modern (task creation is server-directed under SEP-2663).

(The abort-during-poll observation is addressed in the PR reply: the cancel path
uses tasks/cancel and disconnect rejects the in-flight raw request, so the poll
stops; matches legacy.)

Fresh web-UI smoke screenshots (modern connected, task completed, input_required
with the corrected tasks/update note, input completed).

* Tasks modern era: fix uncancelable input-required loop + sticky cancel

Two bug fixes reported during manual testing of the tasks era-fork (#1631):

1. A modern task paused at `input_required` (e.g. `modern_loop_task`, which
never advances) could not be cancelled: the poll loop blocked on the
pending-request modal, so `cancelRequestorTask` sent `tasks/cancel` but the
modal kept re-prompting. Now each modern poll loop registers a per-task
   AbortController (`registerTaskInputAbort`), `cancelRequestorTask` aborts it
to reject the pending elicitation and unblock the poll, and the embedded
input requests are tagged with their owning task id
(`RELATED_TASK_META_KEY`) so the pending-request UI's Cancel action cancels
the TASK rather than sending a "cancel" answer the server would re-prompt on.

2. On the legacy server, cancelling a task briefly showed CANCELLED then
flipped to COMPLETED when the in-flight tool call resolved. Cancellation is
a deliberate terminal decision, so `ManagedRequestorTasksState` now remembers
cancelled task ids and ignores any later non-cancelled status update.

Also: move the compact task id onto its own line in the embedded monitoring
sidebar so the status badge / Cancel Task control don't wrap; DRY the duplicated
task poll-interval logic into a shared `taskPollInterval` helper.

* Refresh tasks PR screenshots + capture both bug-fix scenarios

Recapture the five tasks flow screenshots against the current build and add
two demonstrating the post-review fixes: modern_loop_task cancelled from the
pending-request modal (uncancelable-loop fix) and a legacy task staying
CANCELLED after the in-flight call resolves (sticky-cancel fix).

Claude-Session: https://claude.ai/code/session_01XNdjEPKLG637X8YmhDiEk5

* Address re-review: sticky cancel on legacy refresh + App cancel-routing test

Re-review of the cancelable-loop / sticky-cancel fix raised two non-blocking
hardening items:

1. The legacy tasks/list refresh() rebuilt the list from the server, honoring
   only dismissedTaskIds — so a cooperative-cancel server that re-lists a
user-cancelled task as completed could un-stick the cancel on a manual
Refresh. The refresh path now pins a cancelledTaskIds task to "cancelled"
(keeping it visible-as-cancelled, matching the live-merge handlers) rather
than dropping it. Covered by a new state test.

2. Added an App.test.tsx case locking in the user-facing half of the fix: Cancel
on a task-input-required elicitation calls cancelRequestorTask (not respond).

* Logging: per-request log level on modern servers (era fork) (#1629)

On modern (2026-07-28) servers `logging/setLevel` and the session-scoped
level are gone: the client opts into logs per request via the
`io.modelcontextprotocol/logLevel` `_meta` key, and the SDK does not
attach it automatically, so server logs are silently absent until stamped.

Core (`InspectorClient`):
- `setModernLogLevel(level | undefined)` / `getModernLogLevel()` store the
opt-in level; `mergeMeta` stamps it on every outgoing request, gated on
the negotiated modern era so legacy connections are untouched.
- Cleared on disconnect so it can't leak into the next connection.

Web UI:
- `LogControls` forks on `protocolEra`: legacy keeps the `logging/setLevel`
  selector + Set button; modern shows a per-request opt-in Select (Off +
levels, applied immediately) with copy explaining logs arrive on the
  originating request's stream.
- Threaded `protocolEra` / `modernLogLevel` / `onSetModernLogLevel` through
  LoggingScreen and InspectorView from App.

Tests: unit coverage for the modern LogControls/LoggingScreen fork;
integration coverage proving the `logLevel` `_meta` is stamped on a real
modern server once opted in (and not on legacy).

* test-servers: logging era-fork showcase configs + proof screenshots (#1629)

Add two manual-testing configs and README docs for the logging era fork:

- logging-legacy-http.json: plain streamable-HTTP server with `logging: true`
and a `send_notification` tool. The Logs tab shows the legacy session-scoped
"Set Active Level" selector + Set button; calling the tool streams logs.
- logging-modern-http.json: same, with `transport.modern: true`. Connect with
  Protocol Era = Modern and the Logs tab shows the per-request opt-in control;
the client stamps `_meta["io.modelcontextprotocol/logLevel"]` on every
request (visible in the Network tab request body).

pr-screenshots/ documents the verified behavior end-to-end against both real
servers: the legacy control with logs streaming, the modern per-request
control, and the modern tools/call request body carrying the stamped logLevel
_meta.

* test-servers: send_notification emits via per-request notifier so modern logs stream (#1629)

The `send_notification` tool sent its `notifications/message` through the
global `server.server.notification()`. On the modern (2026-07-28) stateless
leg that isn't tied to the in-flight request, so the SDK answered the tool
call with plain JSON and dropped the log — nothing reached the Inspector's
Logs panel.

Prefer the per-request `extra.sendNotification` (rebuilt from
`ctx.mcpReq.notify`): scoped to the current request, it makes the SDK upgrade
the response to SSE and stream the log back on the originating request's
stream. Falls back to the global notifier (legacy path / no per-request
context). Verified end-to-end: opting in via the Logs-tab "Log Level per
Request" control and calling send_notification now streams the entry on the
modern server; legacy behavior unchanged (server-notification integration
tests green).

README documents the modern logging showcase and the caveat that this test
server streams whenever a tool emits — the SDK beta strips the per-request
envelope from the handler context, so the tool can't gate on the client's
logLevel opt-in (the client-side `_meta` stamp #1629 owns is still fully
visible in the Network tab).

* test-servers: send_notification uses SDK request-scoped log() → spec-faithful modern gating (#1629)

The previous fix routed `send_notification` through `extra.sendNotification`
(`ctx.mcpReq.notify`), which streams on the modern leg but emits
unconditionally — the log showed up even when the client hadn't opted in via
the per-request `logLevel`.

The SDK already provides the correct primitive: `ctx.mcpReq.log(level, data,
logger)` (added in `McpServer.buildContext`). It applies the era-correct
threshold gating itself — on the modern (2026-07-28) leg it reads the
per-request `logLevel` opt-in from the request envelope and drops the message
when the client didn't opt in or the level is below the requested severity; on
legacy it honors the session level from `logging/setLevel` — and emits through
`notify`, so the modern response still upgrades to SSE and the log rides the
originating request's stream.

Surface it as `extra.log` (`toHandlerExtra` maps `ctx.mcpReq.log`) and have
`send_notification` prefer it, falling back to the global notifier for callers
without per-request context. Verified end to end: opting in via the Logs-tab
"Log Level per Request" control stamps `logLevel` in the request `_meta` and
streams the log; setting it back to Off omits the key and the server gates the
log (plain JSON response, no notification). Legacy server-notification
integration tests still green. (No SDK change needed — the earlier apparent
gap was our test server using the wrong low-level API.)

* web+core: add "Log Level per Request" as a per-server setting, defaulting to on (#1629)

Adds the modern per-request log level as a persisted Server Settings option
(Options → "Log Level per Request", below Protocol Era), so a server can opt
into logs by default instead of the Logs-tab control starting at Off.

- types.ts: `ModernLogLevel` (`"off"` | the 8 levels), `DEFAULT_MODERN_LOG_LEVEL`
= "debug" (default-on so a modern connection surfaces server logs out of the
box), `MODERN_LOG_LEVELS`, and an `isModernLogLevel` guard. Field added to
  `InspectorServerSettings` and `StoredMCPServer`.
- serverList.ts: round-trip the field (lift on read, omit-on-default on write,
drop unknown literals) mirroring `protocolEra`; add to the persisted field
  allowlist.
- remote/node/server.ts: validateSettings accepts/validates it and the
normalize path drops a malformed literal.
- inspectorClient.ts: the constructor seeds `modernLogLevel` from the setting
(`"off"` → not opted in), so a modern connection stamps the level from the
first request without UI interaction. mergeMeta still gates on the modern era.
- ServerSettingsForm/Modal: a Select (Off + 8 levels) with a modern-era
description. App seeds the live Logs-tab control from the setting on connect
so the two stay in sync.

Tests: form Select (default/onChange/off/reflect), modal handler, serverList
round-trips, route validation (accept/reject/drop), and constructor seeding
(seeded level stamped from the first request; "off" opts out). The prior
modern-era "stamps once set" test is updated for the new default-on behavior.
Coverage gate green.

* Fix CI + review: format ServerSettingsForm, drop stray network export, broaden ignore

- Prettier-format ServerSettingsForm.tsx (the format:check failure that broke
CI — my pre-push `npm run format` from the repo root was a silent no-op since
there is no root format script; formatting is per-client).
- Remove `inspector-network-logging-modern-2026-07-19T15-11-27.013Z.json`, a
Network-tab export debugging artifact accidentally committed in ca2aa526
  (flagged by review).
- Broaden the .gitignore debugging-artifacts section with
`/inspector-network-*.json` so future network exports are ignored (the
existing `/inspector-history-*.json` didn't match the network export naming).

Verified with the full `npm run ci` (validate → coverage → smoke → storybook),
green across all clients.

* test: add modern-era log delivery + gating e2e tests (#1629)

The modern-era suite covered the client-side `_meta` logLevel stamp but not
that a log actually flows back — the legacy suite already has that
(`should receive server-initiated notifications via SSE/streamable-http`), so
the modern side was the gap.

Add two live e2e tests against a modern (`createMcpHandler`) server serving the
`send_notification` tool with `logging: true`:
- opted-in (server setting = a level): calling the tool delivers a
`notifications/message` over the request's SSE stream, with the right
  level/logger/data.
- not opted in (setting "off"): the tool still returns a result, but the server
gates the log — no `notifications/message` is received.

This exercises the full path end to end: the client seeding + `_meta` opt-in,
and the server's `send_notification` → `ctx.mcpReq.log` era-correct gating.

Verified with the full `npm run ci` (green across all clients).

The per-request log level is meaningless on a legacy server (which uses
logging/setLevel), so hide it in Server Settings when the era is legacy:
- protocolEra pinned to "legacy" → hidden (no connection needed), or
- protocolEra "auto" that negotiated legacy at connect time → hidden.

It stays visible for a pinned "modern" server, and for an "auto" server that is
either not yet connected (era unknown) or resolved to modern.

Threads the negotiated era down: App passes `negotiatedEra` to
ServerSettingsModal only when the settings target is the live-connected server
(`connectionStatus === "connected" && settingsModalTargetId === activeServerId`);
the modal forwards it to ServerSettingsForm, which gates the control on
`configuredEra === "modern" || (configuredEra === "auto" && negotiatedEra !== "legacy")`.

Tests: visibility matrix on the form (legacy hidden; auto+legacy hidden;
auto+unconnected and auto+modern and modern shown) and a modal hide case; the
existing modern-log tests now base on a modern-era settings object. Verified in
the browser (legacy hides, switching to modern reveals it reactively) and with
the full `npm run ci` (green across all clients).

Thread cacheMode through managed list refresh via SDK cache-aware verbs (#1721) (#1723)

* Thread cacheMode through managed list refresh via SDK cache-aware verbs (#1721)

Manual/auto refresh of tools/resources/prompts now forces a cache-bypassing
round trip on modern servers, delivering the raw-request cacheMode control
deferred from #1628.

The InspectorClient list/read path uses the SDK's single-page `client.request`
verbs (for pagination debugging), which never consult the SDK response cache —
so `cacheMode` was inert there. This adds a separate aggregate path that routes
through the SDK's cache-aware high-level verbs, where `cacheMode` is honored:

- InspectorClient: new `listAllTools/Prompts/Resources/ResourceTemplates(
  { cacheMode, metadata })` that call `client.listTools()` etc. (all-page
  aggregate + SEP-2243 x-mcp-header exclusion + cache consult/write). A shared
  `getCacheableRequestOptions` / `aggregateListParams` back all four.
- InspectorClientProtocol: the four `listAll*` methods added.
- ManagedListState: delegates page walking to the SDK verb (config `fetchPage`
  loop → `fetchAll`); `refresh(metadata?, cacheMode?)` threads cacheMode. A
  `list_changed` auto-refresh bypasses the cache (`cacheMode: "refresh"`).
- Managed hooks: user-initiated `refresh()` passes `cacheMode: "refresh"`, so
the existing per-tab Refresh buttons become genuinely cache-bypassing — no
App wiring change. Connect-time load stays default (`'use'`).

The single-page `listTools(cursor)` primitive is unchanged; the paged hooks
still surface per-page `nextCursor`.

* Single-page list pagination: sidebar toggle + server setting (#1721)

Delivers the deferred single-page fetch UI: Tools/Resources/Prompts can be
loaded one page at a time instead of auto-aggregating every page — a defensive
option for servers with very large lists.

- New `singlePageLists` server setting (persisted, server-wide), plumbed through
  ServerConfig/InspectorServerSettings, serverList convert/default, the node +
remote settings paths, and the Server Settings form ("Fetch Lists One Page at
  a Time").
- New `ListPaginationControls` element (a "Single page" switch + "Load next
page" button + "N pages loaded" status) rendered in each list sidebar; the
toggle edits the setting live (optimistic) and persists it.
- The paged state stores (PagedTools/Prompts/ResourcesState) now own loading:
they auto-load page 1 on connect in single-page mode and track nextCursor +
page count as observable state; the managed stores skip their all-page walk
in single-page mode (so nothing is over-fetched) while still lighting the
list-changed indicator. `usePaginatedList` selects the display source and
pagination controls per mode — pure, no data-loading in React effects.
- Refresh is mode-aware: reload page 1 in single-page mode, refetch the whole
aggregate otherwise. Sidebars widened 340→360 so the controls don't crowd
  list entries.
- Test server: `test-servers/configs/pagination-http.json` (12 tools/resources/
prompts, page size 4 → 3 pages each) exercises the flow end to end.

* Address @claude review: template load, double-click guard, auth recovery (#1721)

- Fix: resource templates never loaded on connect in single-page mode. The
connect-time all-page skip now applies only to lists with a paged counterpart
(new `deferInSinglePage` flag on ManagedListConfig; false for resource
templates), so templates still aggregate on connect regardless of the setting.
- Guard `PagedTools/Prompts/ResourcesState.loadPage` against a concurrent load
(fast double-click on "Load next page") so the same page can't be appended
twice; the in-flight load owns the cursor and the second call is a no-op.
- Thread ambient auth recovery through the single-page refresh, Load-next-page,
and mode-switch loads (they previously bypassed it) — usePaginatedList's
onRefresh/onLoadMore now return their promises so App can wrap them.
- Drop the misleading "aggregated list" wording from the paged-store docs.

Tests added for each; npm run coverage green.

* Review round 2: revert optimistic toggle when persisting the setting fails (#1721)

Non-blocking review follow-up. If updateServerSettings rejects, the optimistic
`singlePageListsOverride` is now cleared and the live client setting rolled back
(the effect only clears the override when the persisted value changes, which
doesn't happen on failure), so the UI and client reflect what's on disk rather
than the failed edit. Added an App test for the revert path.

* Fix single-page Refresh not clearing list-changed indicator + UI polish (#1721)

Round-3 review fix (blocking): in single-page mode a `list_changed` lights the
list-changed indicator, but the single-page Refresh reloads page 1 of the paged
store and bypassed `useManaged*.refresh()`, so the indicator was never cleared.
Expose `clearListChanged` from useManagedTools/Prompts/Resources and call it in
the single-page branch of onRefreshTools/Prompts/Resources. Tests added.

UI polish (user feedback on the sidebar control):
- Rename the switch label "Single page" → "Paginated".
- Put "Load next page" on the same row as the switch, space-between (switch
  fully left, button fully right).
- Move the "N pages loaded" status to its own line below the row, centered.
- Hide the whole control once the list is known to be a single page
(single-page mode, page 1 loaded, no nextCursor) — nothing to paginate.

pr-screenshots refreshed to the new layout.

* Rename the singlePageLists setting to paginatedLists for clarity (#1721)

`singlePageLists` read backwards from the "Paginated" switch label (it sounds
like paginated=false). Rename the server setting to `paginatedLists` with the
same value meaning — true = paginated/one-page-at-a-time, false = aggregate-all
— and follow through on the related identifiers for consistency:

- Setting field `paginatedLists` across ServerConfig / InspectorServerSettings,
serverList convert/default, node + remote settings paths, and the form/modal
  callbacks (onPaginatedListsChange).
- `usePaginatedList` / `ListPaginationControls` prop `singlePage` → `paginated`,
  `onSinglePageChange` → `onPaginatedChange`.
- `ManagedListConfig.deferInSinglePage` → `deferWhenPaginated`.
- App state (paginatedListsOverride, onTogglePaginatedLists, …) and all
  tests/fixtures + mode-referring comments.

"single-page" wording that denotes the SDK's one-page `client.request` verb
(inspectorClient) is intentionally left as-is. Purely a rename — no behavior
change; unit + integration + cli + tui + coverage all green.

* Network: modern Mcp-* headers, new error codes, cancellation-as-abort (#1628)

Upgrade the Network tab for the modern Streamable HTTP transport
(SEP-2243 / SEP-2575):

- Recognise and highlight the mirrored headers (Mcp-Method, Mcp-Name,
  Mcp-Param-*, MCP-Protocol-Version), decode =?base64?…?= sentinel values
inline, and cross-check each against the request body so a HeaderMismatch
is visible before the server rejects it.
- Show the new spec error codes distinctly: -32020 HeaderMismatch,
  -32021 MissingRequiredClientCapability, -32022 UnsupportedProtocolVersion
(with the server-advertised `supported` versions), and modern -32601
MethodNotFound on HTTP 404 (distinguished from a legacy bare 404 by its
  JSON-RPC body).
- Reflect cancellation as a connection abort ("Cancelled") rather than a
  hard error, per SEP §7.5.
- Confirm and document browser-path header mirroring: the SDK builds the
Mcp-* headers client-side and the /api/fetch proxy forwards them verbatim
  (new integration test).

New pure util `mcpNetworkHeaders.ts` holds the recognition/decode/classify
logic; new `McpErrorBadge` element renders the distinct error chips. The
optional raw-request controls (cacheMode bypass/refresh, single-page fetch)
are split to #1721 — they are cross-cutting core + feature-tab work beyond
the Network tab.

Also refreshes the AGENTS.md board Status option-ID table (the Building /
MCP Apps Extension columns were removed).

Closes #1628

* Network smoke-test showcase + fix redaction clobbering JSON-RPC error codes

Adds a composable "modern-network" showcase server for exercising the #1628
Network-tab work end-to-end, and fixes a real bug the smoke test surfaced.

Bug fix (core): the fetch-tracking body redaction masked ANY field named
`code` (targeting the OAuth authorization code), including the numeric
JSON-RPC `error.code`. That destroyed the field the Network tab classifies
the modern spec errors on, so -32020/-32021/-32022/-32601 never rendered
against real traffic. Redaction now only masks STRING values of a sensitive
field (secrets are always strings; a numeric code is never one).

Test-server showcase (test-servers): `modern-network-http.json` serves a
`get_weather` tool with an `x-mcp-header: "City"` annotation plus four
`trigger_*` tools. A new `transport.modern.injectSpecErrors` flag installs a
middleware on the modern leg that answers the triggers with real HTTP status
+ JSON-RPC error bodies (400/-32020, 400/-32021, 400/-32022 w/ `supported`,
404/-32601), so the Inspector's error rendering can be driven live.

Mirroring finding (verified live, comment corrected in createRemoteFetch.ts):
the SDK builds the STANDARD mirrored headers (Mcp-Method / Mcp-Name /
MCP-Protocol-Version) unconditionally — they reach the server through the
web proxy — but gates `Mcp-Param-*` on a non-browser environment
(`detectProbeEnvironment() !== "browser"`), so the web client never emits
them. An x-mcp-header tool is therefore uncallable from the web client
against a strict modern server (it answers -32020); it works from the Node
CLI/TUI. The earlier "mirroring works through the proxy" note was corrected.

* Network: move the spec-error badge off the crowded compact top row

In the narrow monitoring sidebar (embedded layout) the McpErrorBadge shared
the top row with the timestamp/method/category/duration/status chips, which
truncated the method and category ("PO…", "TRANSP…") and clipped the error
name. Render the badge separately from `metaBadges` and place it on the
second row, to the right of the URL, so the top row stays readable and the
error name shows in full. The full-width layout keeps it in the top-right
cluster (where there's room).

Claude-Session: https://claude.ai/code/session_01XNdjEPKLG637X8YmhDiEk5

* Network: keep the spec error name's casing in McpErrorBadge (no uppercase)

The four spec/SDK error identifiers are PascalCase (`HeaderMismatch`,
`MissingRequiredClientCapability`, `UnsupportedProtocolVersion`,
`MethodNotFound`). Mantine's Badge uppercases label text by default, which
flattened them to a hard-to-read run-together string
("UNSUPPORTEDPROTOCOLVERSION"). Set `tt="none"` so the badge shows the
spec's own casing with word boundaries intact; short badges (POST, status)
keep the default uppercase.

Claude-Session: https://claude.ai/code/session_01XNdjEPKLG637X8YmhDiEk5

* Network: in the full-width layout, move the spec-error badge to the second row

The full-width entry kept the McpErrorBadge in the top-right cluster, where a
long spec name (e.g. "MissingRequiredClientCapability") crowded the top row
and truncated the method chips and URL. Move it to the start of the second
row (the expand-toggle row) so the top row stays readable; the toggle stays
at the end (`space-between` only when the badge is present). The compact
sidebar already places it on its second row beside the URL.

* Protocol: show Replay as the right-side icon in the wide layout too

The compact (sidebar) ProtocolEntry already renders Replay as an icon next to
the pin/expand icons; the wide layout used a left-aligned "Replay" text
button instead. Make the wide layout match — Replay as the ReplayButton icon
at the start of the right-side action cluster (Replay, Pin, Expand) — so the
two layouts are consistent. Drops the now-unused SubtleButton constant.

* Move the modern spec-error chip + alert from the Network tab to the Protocol tab

The Network tab is focused on the HTTP transaction; the distinct protocol-error
taxonomy (-32020/-32021/-32022/-32601) belongs with the protocol messages. So:

- ProtocolEntry now renders the McpErrorBadge chip on the top line (right after
the direction badge, both layouts) plus an explanatory alert in the expanded
detail, classified by JSON-RPC error code via `classifyProtocolSpecError`.
- NetworkEntry drops the chip + alert; it keeps the HTTP-level features (status
badge, Mcp-* header highlight/sentinel-decode, header/body mismatch marker,
  cancellation-as-abort).
- The SDK delivers -32020/21/22 (HTTP 400) as JSON-RPC error responses, so they
reach the Protocol log directly. -32601 (HTTP 404) and other non-enveloped
errors are *thrown* — the request stays pending. `enrichProtocolEntries`
(App) correlates the Protocol log to the Network log by JSON-RPC id (parsed
from the transport fetch's requestBody) and folds the thrown error onto its
pending request, so it too appears in Protocol. This is a derived view; the
  underlying log is not mutated.

Verified live against the modern-network showcase: both a -32020 error response
and a thrown -32601 render distinctly in the Protocol tab.

Follow-up: a "reveal in Network" link on the Protocol alert (cross-tab
scroll-to + expand of the correlated HTTP entry).

Claude-Session: https://claude.ai/code/session_01XNdjEPKLG637X8YmhDiEk5

* Protocol: show the spec-error chip only in the wide layout, after the method

Refine the placement of the spec-error chip added in the previous commit:
- Wide layout: move it from after the direction badge to right after the
method chip (reads as "tools/call -32020 HeaderMismatch").
- Compact sidebar row: drop the chip entirely — the two-line row stays
uncluttered and relies on its ERROR status badge. The explanatory alert
still appears in the expanded detail in both layouts.

Claude-Session: https://claude.ai/code/session_01XNdjEPKLG637X8YmhDiEk5

* Protocol: "reveal in Network" link — jump from a spec error to its HTTP entry

Adds the cross-tab navigation that was the follow-up to relocating the
spec-error rendering. The expanded Protocol alert now shows a "View the HTTP
request in the Network tab" link when the entry has a correlated Network
request; clicking it:

- correlates the Protocol message to its transport fetch by JSON-RPC id
(`revealableMessageIds` gates the link; `correlateFetchEntry` finds the
  target on click),
- clears whatever filter would hide the target (network filterText +
  category, and the embedded monitorSearch),
- switches to the Network view in whichever mode is active — the pinned
column (`pinMonitor`) when the sidebar is open, or the full-screen header
  tab (`onActiveTabChange`) when it's closed,
- scrolls the matching NetworkEntry into view and force-expands it.

The reveal is a one-shot: a new `revealId` threads NetworkScreen →
NetworkStreamPanel → NetworkEntry, where the targeted entry scrolls itself
into view (rAF, after `useScrollMemory`'s restore) + expands, then clears the
signal via `onRevealComplete`. Verified live in both the pinned-sidebar and
closed/full-screen layouts.

Claude-Session: https://claude.ai/code/session_01XNdjEPKLG637X8YmhDiEk5

* Protocol: only show the "reveal in Network" link when it adds value

Gate the link on a new `httpRelevant` flag: show it only when the error is
either thrown by the SDK (its real HTTP response lives only in the Network
log — the Protocol entry is a synthetic fold) OR tied to the HTTP request
headers. A delivered, protocol-only error gets no link, since the raw HTTP
entry adds nothing over the alert.

- -32020 HeaderMismatch → link (the mirrored headers are the problem)
- -32601 MethodNotFound → link (thrown; the 404 response is only in Network)
- -32021 MissingRequiredClientCapability → no link (delivered, protocol-only)
- -32022 UnsupportedProtocolVersion → no link (supported list is in the alert)

* Network: cite SEP-2575 for cancellation-as-abort, not the internal doc's §7.5

The cancellation note referenced "(SEP §7.5)", but §7.5 is a section of the
internal v2_new_spec_impact.md doc, not a SEP. The behavior — closing the
Streamable HTTP request stream is the cancellation signal; notifications/cancelled
is stdio-only — is defined in SEP-2575 (stateless-mcp). Cite SEP-2575 in the
user-facing alert and the two related comments.

* Network: fix reveal one-shot canceling its own scroll; review doc/comment nits

Address @claude review of #1722:
- Reveal one-shot: move onRevealComplete() inside the rAF (after scrollIntoView)
so clearing revealId (which flips revealed->false and runs the effect cleanup)
can no longer race/cancel the pending scroll. Add a flip-back test.
- Comment the intentional double-mount of revealId (pinned monitor + full-screen
ScreenStage both match; clear is idempotent).
- Doc drift after the Network->Protocol relocation: McpErrorBadge + the
mcpNetworkHeaders module header now say the spec-error chip lives in Protocol.

Claude-Session: https://claude.ai/code/session_01XNdjEPKLG637X8YmhDiEk5

* Network: clarify redaction array-of-scalars edge + revealableIds superset

Address second @claude review of #1722 (both non-blocking polish notes):
- fetchTracking redaction: note that masking assumes a sensitive value is a
scalar or object; an array-of-scalars under a sensitive key would slip through
(no OAuth/token payload has that shape).
- revealableMessageIds: document it's a superset of the linked entries; the
spec-error/httpRelevant narrowing is applied downstream at render.

* Protocol: gate modern -32601 framing to a genuine 404, not in-band 200

Address third @claude review of #1722. classifyProtocolSpecError recognised
-32601 from the code alone, so an ordinary in-band 'method not found' returned
on HTTP 200 was dressed up as the modern MethodNotFound taxonomy: the spec
badge, a 404-asserting alert, and a 'reveal in Network' link that jumped to a
plain 200 (claiming diagnostic value that wasn't there).

Gate the modern -32601 treatment on the correlated fetch actually being a 404
(mirroring classifyMcpSpecError). -32020/-32021/-32022 are SEP-reserved and
unaffected. The correlated HTTP status is threaded from InspectorView (new
correlatedFetchStatusById) through ProtocolScreen/ProtocolListPanel to
ProtocolEntry; when unknown, -32601 falls back to code-only recognition (the
thrown-404 path always carries a 404). Badge, alert copy, and link now resolve
together, so a plain in-band -32601 falls through to ordinary error rendering.

Claude-Session: https://claude.ai/code/session_01XNdjEPKLG637X8YmhDiEk5

* Protocol: require a genuine 404 for modern -32601 (drop unknown-status fallback)

Address fourth @claude review of #1722. The prior fix only rejected -32601 when
the status was *known* and not 404; an unknown status fell back to code-only
recognition. But the thrown-404 path always carries a 404 (folded in from a 404
fetch), so the unknown-status branch is only ever reached by an ordinary in-band
-32601 with no correlated 404 — most notably a stdio connection (no HTTP at all)
— which then wrongly got the modern MethodNotFound badge and 404-asserting copy.

Drop the escape hatch: -32601 is the modern taxonomy only on httpStatus === 404.
Nothing intended is lost (the genuine case always has its 404). Update the tests
(code-only/undefined -32601 now expects null; add a stdio no-correlated-fetch
ProtocolEntry case) and mark the MethodNotFound story with correlatedHttpStatus
404.
